Fysioterapeuttien ja perheiden käsityksiä CP-vammaisten lasten ja nuorten fysioterapeuttisesta ohjauksesta botulinumtoksiini A-injektion yhteydessä

Abstract

Opinnäytetyön tarkoituksena oli nykyisiä fysioterapiakäytänteitä kartoittamalla tutkia botu-linumtoksiini A -injektioon tulevan CP-vammaisen potilaan sekä hänen perheensä ohjauksen toteutumista eräässä HUS:n keskussairaalassa. Fysioterapeuttisesta ohjauksesta on tehty tähän mennessä varsin vähän tutkimuksia eikä lasten ja nuorten fysioterapeuttista ohjausta ole Suomessa juurikaan tutkittu. Fysioterapeuttinen ohjaus on osa botulinumtoksiini A -hoitoprosessia ja sen tavoitteena on tukea potilaan ja hänen perheensä selviytymistä jatkohoidosta.

Opinnäytetyö tehtiin osana Potilasohjaus fysioterapiassa –hanketta ja tutkimuksen aiheena oli fysioterapeuttisen ohjauksen toteutuminen lasten ja nuorten fysioterapiassa. Tutkimuksen tavoitteena oli kolmen tutkimuskysymyksen avulla saada tietoa botulinumtoksiini A -hoitoon osallistuneiden CP-vammaisten lasten ja nuorten sekä heidän vanhempiensa käsityksistä fy-sioterapeuttisesta ohjauksesta sekä selvittää, millä tavalla ohjaus tukee jatkohoidon toteutumista kotona. Tutkimuksessa selvitettiin myös ohjaavien fysioterapeuttien näkemyksiä ohjauksen toteutumisesta.

Tutkimukseen osallistui kolme botulinumtoksiini A -injektion saanutta CP-vammaista lasta tai nuorta ja heidän vanhempansa sekä kaksi ohjaavaa fysioterapeuttia. Tutkimusaineisto kerättiin haastattelemalla tutkimukseen osallistuneita teemahaastattelun avulla sekä videoimalla ohjaustilanteet ja kotihaastattelut. Tutkimus oli luonteeltaan kvalitatiivinen ja aineistot analysoitiin aineistolähtöisellä sisällönanalyysillä.

Tulosten perusteella fysioterapeuttinen ohjaus on fysioterapeuttien kuvaamana moniammatillista yhteistyötä, yhteisiin toimintatapoihin perustuvaa, asiakaslähtöistä ja kokonaisvaltaista fysioterapiaa. Haastatteluista nousi esiin fysioterapeuttien positiiviset kokemukset ohjauksesta sekä toisaalta kokemus ohjaustilanteiden haastavuudesta. Fysioterapeuttien mukaan jatkohoidon tärkeyden korostaminen on olennainen osa fysioterapeuttisen ohjauksen sisältöä. Haastattelujen perusteella perheet kokivat, että sairaalassa tapahtuva fysioterapeuttinen ohjaus oli botulinumtoksiini-hoitoprosessia tukevaa sekä moninaista yhteistyötä perheen ja fysioterapeuttien välillä. Näiden lisäksi ohjaus oli perheiden mukaan myös osittain puutteellista.

Jatkohoidon toteutuminen kotona oli perheiden vastuulla. Perheet kertoivat olleensa sitoutuneita jatkohoitoon. Heidän mukaansa ulkoisilla tekijöillä oli vaikutusta jatkohoidon toteutukseen, joten tarvittaessa perheet sovelsivat harjoitteita kotona.

The purpose of this thesis was to survey physical therapy counselling for patients with Cerebral Palsy and their families after receiving botulinum toxin type A -injection. This study was carried out in one of the central hospitals of The Hospital District of Helsinki and Uusimaa (HUS). There is little research on physical therapy counselling. In Finland, studies on children`s and young people`s physiotherapy have hardly been conducted. Physical therapy counselling is a part of botulinum toxin A –treatment process and its aim is to support patients` and their families`coping in further treatment.

Our thesis was a part of The Physical Therapy Counselling –project and concentrated on physical therapy counselling in children`s and young people`s physiotherapy. The aim of this study was to survey experiences of physical therapy counselling with the help of three research questions. Our informants were parents and their children with Cerebral Palsy who participated in botulinum toxin A –treatment. We also surveyed how counselling supports their further treatment at home. In this thesis we also studied how physiotherapists experienced the carrying out of counselling.

Three patients with Cerebral Palsy, their parents and two counselling physiotherapists participated in this study. The data was collected by using thematic interviews and also video-taping the counselling situations and interviews performed at home. The research method in this thesis was qualitative and the gathered information was analyzed by using material-based content analysis.

Based on the findings of this study according to the physiotherapists physical therapy counselling is multi-professional cooperation, based on common procedures, client-orientedness and comprehensive approach. Based on the interviews, the physiotherapists had positive experiences of physical therapy counselling, but they considered that physical therapy counselling is also challenging. According to the physiotherapists the emphasis on the importance of after care is an essential part of physical therapy counselling content. Based on the interviews, the families experienced that physical therapy counselling in hospital supported the botulinum toxin -treatment process and the versatile cooperation between the family and the physiotherapists. They also thought that the physical therapy counselling was partially inadequate.

Carrying out of further treatment was the families’ responsibility. The families described that they were committed to the further treatment. According to them, external factors had an effect on how they carried out further treatment. Therefore the families applied the exercises at home when needed.
