Kuvat tarinoiksi : Kuvakortit Niuvanniemen sairaalan nuoriso-osastolle

Abstract

Opinnäytetyössä luotiin Niuvanniemen sairaalan nuoriso-osastolle toiminnallisen terapian väline perhe- ja lähisuhdeväkivaltaa kokeneiden lasten ja nuorten terapian avuksi. Nuoriso-osaston henkilökuntaa oli osallistunut syksyllä 2009 perhe- ja lähisuhdeväkivaltaa käsittelevään ammatilliseen PERLA- täydennyskoulutukseen, jonka pohjalta osastolle haluttiin uutta terapiavälineistöä. Opinnäytetyönä tuotettiin akvarellein maalatut kuvakortit ja ohjeet niiden käyttöön. Kuvakorteissa kuvataan sekä positiivisia että negatiivisia tilanteita sadunomaisin keinoin. Opinnäytetyössä on raportoitu kuvien ja ohjeiden toteutusprosessi. Kuvakorttien laatimisessa hyödynnettiin tutkimus- ja muuta asiantuntijatietoa narratiivisuudesta, visuaalisesta lukutaidosta, sadutuksesta ja kuvan käytön terapeuttisuudesta. Tutkimustiedon perusteella tarinallinen työskentelytapa mahdollistaa uuden luomisen ja vanhan uudessa valossa tarkastelun. Nuoren terapiasuhteessa erilaisten kuvitteellisten ja tulevaisuuteen suuntautuvien elämänkerronnallisten tarinoiden kautta voidaan rakentaa toiveikasta uskoa tulevaisuuteen. Kuvakorttityöskentely on yksi väline herättää nuori kertomaan tarinaa elämästään ja pohtimaan tulevaisuuttaan. Kuvan käytön terapeuttisuus perustuu kuvan välityksellä luotavaan vuorovaikutussuhteeseen sekä kuvan tunteita ja muistoja herättävään voimaan. Vaikeista asioista keskusteleminen ja puheeksi ottaminen helpottuu kuvallisen välineen avulla. Toimintaterapeuttisena viitekehyksenä käytettiin Kanadan mallia (Canadian Model of Occupational Performance, CMOP), jossa määritellään, kuinka toiminnallinen suoriutuminen kehittyy yksilön, toiminnan ja ympäristön välisistä vuorovaikutussuhteista. Kuvakorttityöskentelyssä yksilöllä on mahdollisuus tarkastella toiminnallista suoriutumistaan, eri elementtien välisiä vuorovaikutussuhteita ja muutosta eri osa-alueiden välillä. Opinnäytetyönä tuotettu toiminnallinen terapiaväline on Niuvanniemen sairaalan nuoriso-osaston tarpeisiin vastaava ja hyödynnettävissä osaston lasten ja nuorten toiminta-terapiassa ja omahoitajasuhteessa.

The thesis consists of creating a tool of occupational therapy for the Niuvanniemi hospital’s youth ward to support the therapy of children and teenagers who have been subjects to domestic violence. During the fall of 2009 some members of the youth ward staff participated in the vocational supplementary education PERLA, which dealt with domestic violence, thus creating a need for new therapy tools at the ward. As a thesis, a set of picture cards painted with aquarelles were made to-gether with instructions for using them. In the picture cards both positive and negative situations are depicted in fairytalelike manner. The process of creating these pictures and instructions is reported in this thesis. Researches and various other specialist knowledge and findings on narrativity, visual read-ing skills, story crafting and the therapeuticality of using pictures were made use of in the designing of the picture cards. Based on research information, a narrative way of working enables creating new and looking at the old from a different perspective. During a young person’s therapy relationship re-flecting the future using different fictional stories and prospective life stories, hopeful trust in the future can be built. Working with picture cards is a tool to stimulate the child to tell his or her own life stories and to consider the future. The therapeuticality of using pictures is explained by the interaction created with the picture and also by the power to inspire feelings and emotions an image can have. Discussing difficult issues and starting the conversation becomes easier when one has a picture as an instrument. CMOP, or the Canadian Model of Occupational Performance was applied as the occupa-tional therapy’s frame of reference in this thesis. In the CMOP it is defined how the occupational per-formance develops through interaction between the individual, action and environment. Working with picture cards creates an opportunity for the individual to study one’s occupational performing, the interactions between different elements and the change between different sections. The occupational therapy tool produced in a thesis is corresponding to the demands of the Niuvanniemi hospital’s youth ward and it can be made use of with the ward’s children and youths in their occupational therapy treatment and with their primary care nurses.
