Yksiköiden väliset potilassiirrot : gastrokirurgisten ja urologisten potilaiden kokemuksia

Abstract

Potilassiirtoja tehdään päivittäin sairaalan sisällä eri yksiköiden välillä. Sairaanhoitaja on keskeisessä roolissa potilassiirtojen toteutuksessa. (Jauhiainen 2009.) Tämän tutkimuksen tarkoituksena oli selvittää, miten gastrokirurgiset ja urologiset potilaat kokevat potilassiirrot eri yksiköiden välillä. Tutkimuksen tavoitteena on tulosten perusteella kehittää potilassiirtoja tulevaisuudessa.

Tutkimus on osa Tulevaisuuden sairaala - Hoitotyön kehittämisprojektia 2009-2015. Projekti on osa Varsinais-Suomen Sairaanhoitopiirin (VSSHP) T-sairaalan toiminnan kehittämishanketta (T-Pro). Kehittämishankkeessa toimitaan tulevien toimialueiden pohjalta. Toimialueiden tavoitteena on tuoda palvelut potilaan lähelle. (Korvenranta & Särkkä 2010.)

Tutkimusongelmia oli kaksi. Ensimmäinen etsi vastausta kysymykseen miten gastrokirurgiset ja urologiset potilaat kokevat siirrot yksiköstä toiseen. Toinen ongelma pyrki selvittämään kokevatko potilaat ongelmia siirtoihin liittyen, jos niin minkälaisia. Tutkimus toteutettiin kvalitatiivisesti haastattelemalla potilaita (N=11) Turun yliopistollisen keskussairaalan gastrokirurgian ja urologian osastoilla. Haastattelut toteutettiin teemahaastatteluina. Aineisto litteroitiin ja analysoitiin sisällönanalyysillä. Aineistosta etsittiin ilmaisuja, jotka vastasivat tutkimusongelmiin. Ilmaisut pelkistettiin ja pelkistetyt ilmaukset ryhmiteltiin ja luokiteltiin yleisempään muotoon.

Tulosten perusteella potilaat olivat suhteellisen tyytyväisiä siirtoihin eri yksiköiden välillä. Siirroissa ilmenneet ongelmat pelkistettiin kolmeen alaluokkaan: kivuliaisuus siirron aikana, fyysisen ympäristön ongelmat ja hoitohenkilökunnan toimintaan ja resursseihin liittyvät ongelmat. Fyysisen ympäristön ongelmiin kuului käytävien epätasaisuus ja epämiellyttävät tuntemukset siirron aikana. Hoitohenkilökuntaan ja resursseihin liittyi kiire, tiedottamisen riittämättömyys ja odottaminen.

Inter-hospital patient transfers occur daily between different units. Nurse is in a key role in executing patient transfers. The purpose of this study was to describe how gastrosurgical and urological patients experience patient transfers between different units. The aim of this study was to find out do patients experience problems related to patient transfers and if they do, what kind of problems.

The study is a part of Future hospital - Nursing development project 2009-2015. The project is a part of the new T-hospitals development project (T-pro) in Intermunicipal Hospital District of Southwest Finland. The T-pro -project will consist in process based units. The aim of these units is to bring the services near to the patient. (Korvenranta & Särkkä 2010.)

There were two research questions. In the first question the aim was to describe how gastrosurgical and urological patients’ experience their transfers between units. In the second question the aim was to find out do patients’ experience problems related to patient transfers and if they do, what kind of problems. The approach of this study was qualitative and data was collected by interviewing patients (N=11) in gastrosurgical and urological units in Turku University hospital. Interviews were semi-structured. Data was transcribed and analysed with content analysis. Analysis was done by finding expressions that answered to the two research questions that were set. Expressions were simplified and simplified expressions were put into groups and classified into more extensive forms.

Based on the findings, patients were relatively satisfied with patient transfers between units. Problems that patients experienced during transfers were grouped into three classes: pain during a transfer, problems in physical environment and in nursing staff’s actions and resources. Physical environments problems included the roughness of corridors and unpleasant feelings during transfer. Busy atmosphere, lack of information about transfer process and waiting times within transfers were part of problems related to nursing staff’s actions and resources.
