Gastrostoomaa tarvitsevan lapsen hoitotyö

Abstract

Tämän toiminnallisen opinnäytetyön tarkoituksena oli tuottaa sairaanhoitajaopiskelijoille itsenäisesti suoritettava verkkokurssi gastrostoomaa tarvitsevan lapsen hoitotyöstä. Verkkokurssi tuotettiin Tampereen ammattikorkeakoululle, joka toimii opinnäytetyön yhteistyötahona. Opinnäytetyön tuotoksen tavoitteena on lisätä sairaanhoitajaopiskelijoiden ymmärrystä ja osaamista gastrostoomaa tarvitsevan lapsen hoitotyön osalta.

Gastrostooman, eli perkutaanisen endoskooppisen gastrostooman, avulla voidaan turvata ravitsemustila. Gastrostooman avulla voidaan toteuttaa myös lapsen lääkehoitoa. Sen asennusta harkitaan silloin, kun lapsi tarvitsee letku-ravitsemusta vähintään 6 viikon ajan. Opinnäytetyössä on myös kerrottu tyypillisempiä syitä gastrostooman asennuksesta ja sen komplikaatioista lapsella.

Gastrostooman hoidossa tulee ottaa huomioon aseptiikka ja turvallisuus gastrostoomaa hoidettaessa päivittäin. Vanhempien kokemuksia olemme tuoneet esille erilaisten tutkimusten sekä myös opinnäytetyötä varten haastatellun vanhemman esiin tuomien asioiden avulla.

Verkkokurssi tehtiin Tampereen ammattikorkeakoululle Moodle-oppimisalustalle. Verkkokurssi koostuu tekstiosuuksista, kuvista sekä lyhyistä video-osuuksista. Sen lisäksi verkkokurssin lopussa on testi, jonka avulla opiskelijat varmistavat oppimisensa. Opinnäytetyötä varten on haastateltu kolmea vanhempaa.

Kehittämisehdotuksina esitetään oppaan tekemistä kouluterveydenhuollon ja terveyskeskuksien hoitajille sekä vastaavanlaisen verkkokurssin tekemistä aikuispotilaan kohdalla.

The purpose of this thesis was to produce an independent online course for nurse students about nursing a child in the need of a gastrostomy. The online course was produced for the Tampere University of Applied Sciences which acts as a co-operative institution for the thesis. The aim of the thesis is to add to the nurse students’ understanding and knowledge on the nursing practice with a child in the need of a gastrostomy.

With gastrostomy, or percutaneous endoscopic gastrostomy, the patient’s nutrition levels can be ensured. Also, a child’s pharmacological treatment can be executed with the help of a gastrostomy. The installation of gastrostomy is considered when the child needs tube feeding for at least six weeks. In addition, the thesis provides more typical reasons for the installation of gastrostomy and the complications it can cause to a child.

In the nursing care of gastrostomy, aseptic and safety have to be taken into account daily. The parents’ experiences have been introduced by various research, as well as by analysing the interview material that an interviewee parent provides.

The online course was produced for Moodle -learning platform of the Tampere University of Applied Sciences. The course consists of articles, pictures and short video clips. In addition, the course finishes with a test which will allow the students to prove their learning outcomes.

For the future research, it is suggested that the there would be educational material of children’s gastrostomy for the nurses working in school health services or health centres, as well as a similar online course for the treatment of an adult patient in the need of a gastrostomy.
