Seniori-Vamos toimintamallin kehittäminen Vuosaaren alueella

TIIVISTELMÄ

Pirjo Lindholm Seniori-Vamos -toimintamallin kehittäminen Vuosaaren alueella 68, 3 liitettä Syksy 2020 Diakonia-ammattikorkeakoulu Sosiaali- ja terveysalan YAMK Sosionomi (YAMK) Johtamisen ja yhteiskunnallisesti vaikuttavan palvelutuotannon kehittäminen

Opinnäytetyö toteutettiin kehittämishankkeena Diakonissalaitoksen Seniori-Vamoksen ja Vuosaaren kotihoidon yhteistyössä. Kehittämishankkeen tavoitteena oli kuvata ja mallintaa Seniori-Vamoksen toimintakäytäntöjä, tuottaa tietoa toimintamallin hyödyistä ja kehittämiskohteista erityisesti työntekijöiden näkökulmasta sekä kehittää toimintaa Vuosaaren alueella.

Kehittämishanke toteutettiin toimintatutkimuksena. Kehittämishankkeessa toteutettiin toimintatutkimukselle tyypillinen kehittämisen sykli, joka sisälsi kehittämisryhmätyöskentelyn suunnittelun, toteuttamisen, tulokset ja arvioinnin. Kehittämishankkeen aineisto kerättiin kehittämisryhmätyöskentelyllä. Kehittämisryhmään osallistui Seniori-Vamoksen projektityöntekijöitä ja Vuosaaren kotihoidon työntekijöitä yhteensä 8 työntekijää. Kehittämisryhmä kokoontui kolme kertaa touko-elokuun 2019 aikana. Kehittämishankkeen tuloksien arviointi tehtiin marraskuussa 2020.

Kehittämisryhmässä käsiteltiin Seniori-Vamos toimintaan ja kehittämishankkeen tutkimuskysymyksiin liittyviä teemoja erilaisilla ryhmätyöskentelymenetelmillä. Kehittämisryhmässä käydyt keskustelut nauhoitettiin ja niissä tuotetut kirjalliset materiaalit kerättiin tutkimusaineistoksi. Lisäksi kehittämiskerroista kerättiin palautetta osallistujilta. Kehittämishankkeen aineisto analysoitiin sisällönanalyysillä.

Kehittämishankkeen tuloksista ilmeni etsivän vanhustyön asiakkuutta kuvaavia tekijöitä, Seniori-Vamos toiminnan yhteistyön osa-alueita, Seniori-Vamos toiminnan haasteita ja kehittämisnäkökulmia. Seniori-Vamos toiminnan haasteet liittyvät yhteistyöhön, tiedonkulkuun, tiedon puutteisiin ja asiakkaiden tuen tarpeiden tunnistamiseen. Seniori-Vamos toimintaa voidaan kehittää yhteistyötä, tiedonkulkua, etsivän työn osaamista, resursseja ja kotihoidon tiimin toimintaa kehittämällä. Seniori-Vamos toiminnan ja etsivän vanhustyön kehittäminen edellyttää tiivistä yhteistyötä ja aikaa yhteiselle kehittämiselle.

ABSTRACT

This thesis was implemented as development work with the Seniori-Vamos project of Diakonissalaitos and Vuosaari home care, Helsinki, Finland. The aim of the development work project was to explore operational practices of Seniori-Vamos and its benefits and development sites especially from the employee’s perspective. The main aim was the development of the Seniori-Vamos project in Vuosaari area.

The development work project was carried out as an action research. In this action research a development cycle was implemented, which included the design, implementation, results and evaluation of the development team work. The material was collected in the development team meetings. The development team included eight representatives of the home care and the Seniori-Vamos project. The development team met three times during May-August 2019. An evaluation of the results of the development work project was carried out in November 2020.

The development team dealt with themes related to the Seniori-Vamos project and development work project issues through a variety of group work. The discussions in the development team were recorded and the produced written materials were collected as research material. In addition, feedback from participants was collected from the development team. The research material was analyzed through content analysis.

The results of the development project showed factors describing the clientele of the outreach elderly work, aspects of the cooperation of the Seniori-Vamos project, challenges and development perspectives. Senior-Vamos operation’s challenges are related to collaboration, flow of information, information deficiencies and identifying customers’ support needs. Seniori-Vamos activities can be developed through collaboration, flow of information, knowledge of the outreach elderly work, resources and development of the home care team. The development of Seniori-Vamos operational model and outreach elderly work requires close cooperation and time for development together.
