" Woi Whoike, mikä woihke" : Kirjaamisen kehittämisprosessi

Abstract

Tämän opinnäytetyön toimeksiantajana toimii Forssan Seudun Terveydenhuollon Kuntayhtymän (FSTKY) eräs pilottiosasto, jossa on jo viime vuonna käsitelty rakenteista kirjaamista eNNI-hankkeen myötä. Tämä opinnäytetyö on jatkoa Pirjo Hakalan vuonna 2010 tehtyyn opinnäytetyöhön ”Kohti rakenteista kirjaamista”. Hakala tutki työssään tuloillaan olevaa sähköistä kirjaamista ja sen toteutumista samaisella pilottiosastolla. Tämän kehittämisprojektin tavoitteena on tässä opinnäytetyössä esiintyvän pilottiosaston kirjaamisen kehittäminen. Kehittämisprojektin tarkoituksena on luoda pilottiosaston henkilökunnalle valmiudet käyttää tuloillaan olevaa rakenteista kirjaamistapaa.

Tämä opinnäytetyö toteutettiin koulutussisältöisenä kehittämisprojektina. Osaston kirjaamisen kehittämistarpeiden selvittämiseen käytettiin SWOTanalyysiä. Saadun tiedon pohjalta pilottiosaston henkilökunnalle järjestettiin koulutustilaisuuksia. Kirjaamisen kehittämiseen osallistuneen pilottiosaston tehtäväksi tuli mahdollistaa henkilökuntansa osallistuminen koulutustilaisuuksiin työvuorosuunnittelun ehdoin. Projektin toteuttajien tehtäväksi jäi järjestää puitteet, kuten tilat ja koulutustilaisuudet, rakenteisen kirjaamisen käyttöön soveltuva ohjelmisto ja tietokoneet. Koulutustilaisuuksiin soveltuvat välineet ja tilat mahdollisti Hämeen ammattikorkeakoulun (HAMK) Forssan toimipiste. Koulutustilaisuuksissa käytiin lävitse teorian ja käytännön pohjalta rakenteista kirjaamista hoitotyön prosessin kautta. Potilasesimerkkeinä käytettiin pilottiosastolle tyypillisiä potilastapauksia.

Koulutustilaisuuksia järjestettiin neljä kertaa, ja niiden sisältö muodostettiin pilottiosaston tarpeita vastaavaksi. Kehittämisprojektissa hoitotyön prosessi koettiin teoriassa tutuksi, mutta sen käyttäminen hoitosuunnitelmaa tehtäessä haastavaksi. Eritoten haasteellista oli erottaa hoitotyön prosessissa toisistaan ongelmat, tavoitteet ja keinot, niiden sekoittuessa useasti keskenään. Kirjaamiskoulutukset nähtiin tarpeellisena sekä nyt että tulevaisuudessa.

The client for this thesis is a pilot ward of the Joint Municipal Board for Health Care in Forssa (FSTKY), Finland. This ward dealt with structured nursing documentation already in 2010 in context with the Enni initiative. This thesis continues the topic of Pirjo Hakala's thesis from 2010, Towards Structured Documentation. In her work, Hakala studied electronic documentation, which is making its way to the field of nursing, and how it has been implemented at the above-mentioned ward. The objective of this project is to develop documentation at the pilot ward presented in this thesis. The purpose of the development project is to create abilities for the ward's staff to use the new structured documentation method.

This thesis was done as a development project consisting of training. In order to find out the needs of development in the ward's documentation, we used a SWOT-analysis. Training sessions for the staff were arranged based on the information gathered from the analysis. The pilot ward’s role became to enable its staff's participation in the training sessions in terms of shift planning. The role of the project implementers thus became to make arrangements, such as booking the facilities and times for the training sessions, and choosing the software and computers applicable for structured documentation. Equipment and facilities for the training sessions were provided by the HAMK University of Applied Sciences campus in Forssa. During the training sessions, we addressed structured documentation via the process of nursing, based on both theory and practice, and by using examples of patient care that were typical for the pilot ward. There were four training sessions, and the content of the training sessions was customised according to the pilot ward's needs.

During the development project, the process of nursing was found to be familiar in theory, but to use it as a basis for a treatment plan was regarded as challenge. It was especially challenging to make difference between methods, objectives and problems in the nursing process, as these were often mixed together. Training sessions in documentation were considered necessary for the current situation as well as for the future.
