Hoitajien virtsatiekatetrointiin ja aseptiikkaan liittyvien tietojen ja taitojen kehittäminen : Osastotunti terveyskeskuksen osastojen henkilökunnalle

Abstract

Hoitoon liittyviä infektioita esiintyy Suomessa vuosittain paljon. Infektiot aiheuttavat kustannuksia yhteiskunnalle sekä haittaa potilaalle. Virtsateiden katetrointi aiheuttaa suuren osan hoitoon liittyvistä infektioista. On tärkeää, että hoitohenkilökunnan virtsatiekatetrointi- sekä infektioiden ehkäisytaidot ovat riittävät. Opinnäytetyö toteutettiin yhteistyössä Seinäjoen sosiaali- ja terveyskeskuksen sekä valtakunnallisen STEPPI-hankkeen kanssa. Opinnäytetyön tarkoituksena oli kuvata oikeaoppista virtsatiekatetrointitekniikkaa ja miten hoitoon liittyviä infektioita voidaan ehkäistä sekä hoitajien omia kokemuksia sekä kehittämistarpeita liittyen virtsatiekatetrointiin ja aseptiikkaan. Aiheesta järjestettiin osastotunti Seinäjoen kaupunginsairaalan kolmelle osastolle. Osastotuntien sisältö pohjautui tutkittuun tietoon. Opinnäytetyön tavoitteena oli ohjata hoitohenkilökuntaa oikeaoppiseen virtsatiekatetrointitekniikkaan ja aseptiseen toimintaan. Opinnäytetyö toteutettiin kolmessa vaiheessa: teoreettinen viitekehys, kyselytutkimus osastojen hoitohenkilökunnalle sekä osastotunnit. Kyselytutkimus toteutettiin lokakuussa 2020. Sen pohjalta saatiin tietoa, millaisia asioita hoitohenkilökunta kokee haastavina ja millaista ohjausta he tarvitsisivat. Tieteellisistä lähteistä ja laeista koostetun teorian pohjalta luotiin osastotunneille PowerPoint -esitys, joka esitettiin osastoilla marraskuussa 2020. Osallistujilta kerättiin suullinen palaute. Jokaisella hoitajalla tulee olla valmiudet tehdä aseptisia toimenpiteitä ja organisaation johdon sekä hoitajan itsensä on pidettävä huolta, että tiedot ja taidot ovat riittävät. Oikeaoppisen katetrointitekniikan hallitseminen on tärkeää, ettei vaurioiteta potilaan virtsateitä tai aiheuteta infektioita. Tulevaisuudessa voisi tutkia osastojen hoitoon liittyvien infektioiden yleisyyttä ja sitä, kuinka moni hoitoon liittyvistä infektioista on katetriperäisiä infektioita.

There are many treatment-related infections in Finland every year. Infections cause costs to the society as well as harm to the patient. Urinary catheterization causes a large proportion of treatment-related infections. It is important that nursing staff have adequate urinary catheterization and infection prevention skills. The thesis was carried out in collaboration with the Seinäjoki Social and Health Center and the national STEPPI project. The purpose of the thesis was to describe the correct urinary catheterization technique and how treatment-related infections can be prevented, as well as the nurses' own experiences and development needs related to urinary catheterization and asepsis. A ward teaching session was organized on the topic for the three wards of Seinäjoki City Hospital. The content of the ward lessons was based on the researched information. The aim of the thesis was to guide nursing staff regarding the correct urinary catheterization technique and aseptic procedures. The thesis was carried out in three stages: a theoretical framework, a survey for the nursing staff of the wards and ward teaching sessions. The survey was conducted in October 2020. Based on that, information was obtained on what kind of issues the nursing staff find challenging and what kind of guidance they would need. Based on a theory composed of scientific sources and laws, a PowerPoint presentation was created for the wars teaching sessions, and it was presented in the wards in November 2020. Oral feedback was collected from the participants. Every caregiver should have the capacity to perform aseptic procedures. The management of the organization as well as the caregiver themself, should ensure that the knowledge and skills are adequate. Mastering proper catheterization techniques is important so the patient's urinary tract is not damaged or infected. In the future, the prevalence of wards treatment related infections and how many of them are catheter-related infections could be researched.
