Päiväkirurgisen potilaan ohjaaminen toimenpiteeseen : Potilaan opas Nurmeksen ja Valtimon terveydenhuollon kuntayhtymän päiväkirurgisille potilaille

Abstract

Potilaille tehdään nykyisin yhä enemmän leikkauksia päiväkirurgisesti. Leikkaukseen tuleva poti-las valmistautuu itse toimenpidettä varten ja toteuttaa itsehoitoa sen jälkeen. Ohjauksen antami-nen leikkaukseen tulevalle on tärkeä osa koko perioperatiivista prosessia. Ennen toimenpidettä potilaalle käydään läpi koko leikkausprosessi ja hänen oma osuutensa hoidossa. Hoitotyö on potilaslähtöistä tukemista, opastamista ja valmistamista. Opinnäytetyön tarkoituksena oli preope-ratiivisen hoitotyön kehittäminen. Tehtävänä oli tuottaa toimeksiantajan käytettäväksi potilaan opas, jota voidaan käyttää potilasohjaustyössä apuvälineenä ja potilailla itsehoitoa ohjaavana materiaalina.

Opinnäytetyö toteutettiin toiminnallisena opinnäytetyönä. Potilaan oppaan tarve tuli esille käy-tännön hoitotyön toteuttajilta, ja aineisto suunniteltiin yhdessä koko leikkaustiimin kanssa. Potilaan oppaan teknisestä toteuttamisesta vastasivat opinnäytetyön tekijät. Opas luovutettiin Nurmeksen terveyskeskuksen käyttöön sähköisenä versiona. Siihen on sisällytetty tietoa leikkaukseen valmistautumisesta, leikkauksen aikaisesta toiminnasta sekä jälkihoidosta.

Tehdyn palautekyselyn perusteella potilaan opas koettiin hyödylliseksi, ja se sisälsi tarpeeksi tietoa leikkauspotilaan ohjaamiseksi. Saadun palautteen perusteella muokkasimme vielä ulko-asua tekstiä suurentamalla. Jatkokehittämisaiheena on tutkia, kuinka potilaan opas on vaikuttanut potilaan selviytymiseen ja pelkoihin leikkausprosessin aikana. Kehittämistehtävänä on lisätä hoitotyöntekijöiden taitoa havaita päiväkirurgisten potilaiden yksilölliset ohjaustarpeet.

Outpatient surgery is growing in popularity and patients are treated increasingly with same-day surgery. A patient prepares for the upcoming operation by himself and will implement self-care after the procedure. Pre-operative guidance of the patient is an important part of the periopera-tive process. Prior to the operation, the whole procedure will be declared to the patient and his own role in the after care is explained. Nursing is patient-oriented support, guiding, and prepa-ration. The purpose of this study was to improve preoperative nursing. The task was to provide a patient guide for the commissioner. This guide can be used as a tool in patient education work and as a self-care guide for patients.

The thesis was carried out as a functional thesis. The need for the patient guide was revealed by practical nurses and the material was planned with entire surgical team. The realization of the patient guide was done by us. An electronic version of the guide was handed to the use of the health centre of Nurmes. The guide includes information about how to prepare for the procedure, what will happen during the procedure and how aftercare is done.

Based on the feedback survey the patient guide was found useful, and it contained needed in-formation to guide the surgical patients. After having got some feedback, we enlarged the font size to be more appropriate. A further topic of the research is the impacts of the patient guide on the coping of the patients and on the progress of their fears during the process. There is room for development in increasing employees' skills to detect the patients´ individual guidance needs.
