Turvallinen lääkehoito kotihoidossa : Henkilökunnan ja asiakkaiden kokemuksia lääkehoidosta Espoon kaupungin kotihoidossa

Abstract

Opinnäytetyö käsittelee turvallisen lääkehoidon kehittämistä Espoon kaupungin kotihoidossa Matinkylässä. Opinnäytetyön tavoitteena oli parantaa lääkehoidon turvallisuutta ja sujuvuutta kotihoidossa ja tarkoituksena kehittää lääkehoidon toimintamalleja yhteistyössä kotihoidon henkilökunnan kanssa. Opinnäytetyössä tarkastellaan erityisesti yli 75-vuotiaiden kotihoidon asiakkaiden lääkehoitoa, sekä henkilökunnan kokemuksia lääkehoidon sujuvuudesta. Työ toteutettiin osana Espoon Suurpellon aluetta kehittävää Koulii-hanketta.

Opinnäytetyössä etsitään lääkehoidon turvallisuutta uhkaavia riskitekijöitä kyselytutkimuksen avulla. Vastaajiksi pyydettiin yhteistyötiimin kaikki yli 75-vuotiaat asiakkaat (n=35) sekä tiimin henkilökunta (n=6). Molemmille vastaajaryhmille tehtiin omat kyselylomakkeensa huomioiden iäkkäiden vastaajien erityistarpeet. Asiakkaiden kyselylomake oli luonteeltaan kvantitatiivinen, kun taas henkilökunnan lomakkeeseen laadittiin kvalitatiivisia kysymyksiä.

Täytettyjä kyselylomakkeita palautui asiakkailta 20 ja hoitajilta viisi. Asiakkaiden vastausten perusteella lääkehoidosta ei tullut esille erityisiä riskitekijöitä. Suurin osa asiakkaista koki kotihoidon toteuttaman lääkehoidon olleen virheetöntä, ja lähes kaikki vastaajat olivat saaneet lääkehoidon ohjausta kotihoidon henkilökunnalta. Henkilökunnan vastausten perusteella taas lääkehoidon turvallisuutta ja sujuvuutta riskeeraavat eniten jatkuva kiire, huono tiedonkulku erikoissairaanhoidon ja kotihoidon välillä, lääkehoitoa koskevan kirjaamisen puutteellisuus, yhteistyö apteekin kanssa sekä asiakkaiden lääkelistojen epäajantasaisuus. Hoitajat kuitenkin kokivat hallitsevansa lääkehoidon työtehtävänsä pääpiirteissään hyvin.

Kyselyn tulokset esiteltiin kotihoidon henkilökunnalle yhteisesti järjestetyssä kehittämiskokouksessa. Kokouksessa keskityttiin tarkastelemaan erityisesti sellaisia asioita, joihin kotihoidon henkilökunta voi päivittäisessä työssään vaikuttaa. Esimerkiksi lääkehoidon kirjaamista ja asiakkaiden lääkelistojen säännöllistä päivittämistä voi tehostaa ilman, että työhön tarvitsee tehdä suuria muutoksia. Sen sijaan esimerkiksi erikoissairaanhoidon tai apteekin toimintaan on yksittäisen työntekijän vaikeampi vaikuttaa. Hoitajat toivat kehittämiskokouksessa esille erityisesti kiireen vaikutuksen työhön ja lääkehoidon toteutukseen. Ajanpuutteen koettiin vähentävän myös työn kehittämisen mahdollisuuksia, kun uusien ideoiden käyttöönottoon ja toimintamallien vakinaistamiseen on vaikeaa löytää muiden työtehtävien ohessa aikaa.

Safe medication in home care

This thesis deals with the development of safe medication for the City of Espoo in home care in the Matinkylä region. The objective of this thesis was to increase the safety and ease of medication in home care and also to improve the methods of medication in co-operations with the staff working in home care in Matinkylä. This thesis will focus on the medical treatment for people over 75 years old and in need of home care, and also on the experiences of the home care staff regarding the ease of medication. This thesis was carried out as a part of a project called "KOULII" which is intended for the development of the fairly new and growing region in Espoo called Suurpelto.

To have an understanding of the current situation at the beginning of this thesis, a survey was carried out to reveal the risk factors that were threatening the safety of medication. The respondents were both the clients over 75 years old (n=35) and the staff members (n=6) from the co-operation team in Matinkylä. Both groups of respondents had their own questionnaires and the special needs of the elderly respondents' were considered. The client questionnaire was more quantitative by nature whereas the staff questionnaire was more qualitative. The number of filled out forms was 20 from the clients and 5 from the staff. Based on the answers from the clients there were not any significant risk factors which could be considered a threat to the medication. Most of the clients had experienced the medication provided by home care to be faultless. The staff's answers however showed that the most concerning threat to the safety and ease of medication is caused by the continuous rush, poor flow of information between the specialized medical care and home care, the lack of documentation of the medication, the co-operation with the pharmacy and finally the drug lists which are not up to date. The staff however felt that they are in good control of the overall tasks related to medication.

The results were presented to the home care staff in a co-arranged development meeting. The main focus at the meeting was to examine specific issues which the home care staff can have an impact on in their daily routines at work. For example, increasing the documentation of the client medication and updating the drug lists regularly for the clients are ways to improve the effectiveness of the daily tasks, without making any major changes to the daily routines. The other issues like the problems with the specialized medical care or the collaboration with the pharmacy, are very difficult to influence based on the results of this thesis. At the development meeting the staff discussed especially what impact the constant rush has on the normal work and the overall medication. The lack of time was also considered to be an issue which diminishes the possibilities to improve the daily routines and tasks. It is considered difficult to try and implement new ideas and regularize new procedures when there is no additional time in connection with the daily tasks.
