Sairaanhoitopiirien Taloudellinen Tehokkuus

Abstract

Tämän opinnäytetyön tavoitteena on laatia uusi tapa mitata sairaanhoitopiirien taloudellista tehokkuutta. Sairaanhoitopiirien vastuulla on erikoissairaanhoidon tuottaminen, niillä on yhteiskunnallisesti erittäin tärkeä rooli ja vastuu. Sairaanhoitopiirien taloudellinen ohjaus ja johtaminen vaativat erilaisia mittareita, työkaluja, jotka kukin omista rajoittuneista lähtökohdistaan antavat jonkinlaisen kuvan toiminnan kehityksestä ja tilasta. Työn toimeksianto tuli Etelä-Pohjanmaan Sairaanhoitopiiriltä, joka pyrkii parantamaan toimintansa tehokkuutta.

Tässä työssä etsitään toisaalta uutta, toisaalta mahdollisimman olennaista työkalua talousohjauksen avuksi.

Tämän työn teoriaosuudessa esitellään taloudellisen tehokkuuden mittaamiseen liittyvää termistöä ja taustoja, jotka luovat pohjan käytännön selvitystyölle.

Empiirisessä osuudessa kerrotaan taloudellisen tehokkuuden ideaalisen mittausmallin etsinnästä ja sen ongelmista. Lopuksi edetään toteutuneeseen mittausmallin. Toteutunutta mittausmallia sovellettiin Etelä-Pohjanmaan Sairaanhoitopiirin aineistoihin.

Lopuksi käsitellään toteutuneen mittausmallin tuloksia ja annetaan ohjeistusta tulkintaa ja jatkokäyttöä varten. Lisäksi käsitellään ideaalisen- ja toteutuneen mittausmallin eroja ja syitä eroihin. Tutkimusten aikana kävi ilmi ettei sairaanhoitopiirien toiminnan tehokkuutta voitu laskea. Tästä syystä toteutuneen mittausmallin näkökulmaksi valikoitui tuottavuuden mittaus. Lopuksi tämän työn havaintojen pohjalta ehdotetaan jatkokehityshankkeiksi toimia joilla tehokkuuden mittausta voitaisiin tulevaisuudessa toteuttaa.

Aineistona on Etelä-Pohjanmaan Sairaanhoitopiirin vuosikertomukset ja kirjanpitoaineisto. Tarkastelujaksolla 2009-2011 kaikkien hoitotapojen tuottavuus laski. Tuottavuuden muutosten kanssa tulee olla kriittinen: hoidon vaikuttavuus on vähintään yhtä tärkeä muuttuja, johon tuottavuuden muutokset eivät ota kantaa.

The goal of this thesis is to create a new way for the measurement of hospital districts' economic efficiency. Hospital districts' are responsible for the production of specialized healthcare services, thus they have a big role and responsiblity in the society. Hospital districts' economic guidance and management demand various measuring tools, which each from their limited framework and premises offer some form of an image of the developments and status of the operation. This thesis was ordered by South Ostrobothnia Hospital District, which is striving to improve the efficiency of their operation.

In this work the goal is to look for a new tool that was to be as relevant as possible, to help with the economic management's needs.

The theory part of this work presents terminology related to economic efficiency and its measurement as well as backgrounds that create the basis for this research paper.

The empiric part of this work tells of the search for an ideal way of measuring economic efficiency and the problems that are related to it. Finally the work advances to the realized measurement tool. The realized measurement tool was applied to South Ostrobothnia Hospital District's data.

Finally the work deals with the results of the realized measurement tool and provides guidance for interpretion and further usage. Additionally the work deals with the differences between the realized and ideal measurement tools, as well as the reasons for those differences. During the research it became evident that hospital districts' economic efficiency could not be calculated. Hence this thesis changed the emphasis from efficiency to productivity and its measurement. Finally, based on the findings of this thesis, such follow up studies are proposed which would enable the measurement of efficiency of hospital districts in the future.

Empirical material consists of annual reports and bookkeeping materiel of South Ostrobothnia Hospital District. During 2009-2011 the productivity of all treatment methods fell. Analyzing the results requires criticism: the effectiveness of treatment is at least as importan of a factor, which is not addressed by productivity.
