Reumasairaan lapsen potilasohjaus sairaanhoitajien kokemana

Abstract

Tutkimuksen tarkoituksena oli kuvata reumasairaiden lasten parissa työskentelevien sairaanhoitajien kokemuksia sekä tuntemuksia lapsipotilaan ohjaamisesta. Tarkoituksena oli tuottaa tietoa, jonka avulla pystytään parantamaan potilasohjausta.

Tutkimusmenetelmä oli laadullinen tutkimus. Tutkimuksen induktiivinen aineisto kerättiin teemahaas-tattelun avulla. Teemahaastattelussa käytetyt teemat ja apukysymykset muodostettiin jo olemassa olevasta tiedosta liittyen potilaan ohjaukseen. Haastatteluihin osallistui kolme sairaanhoitajaa, jotka työskentelevät reumasairaiden lapsipotilaiden parissa. Teemahaastattelut toteutettiin lokakuun 2012 aikana. Aineiston analyysi toteutettiin sisällönanalyysin avulla.

Tutkimustulosten mukaan jokaiseen teemaan (Lapsipotilaan ja hoitohenkilöstön konteksti; Lapsen kogni-tiivinen kehitys; Lapsipotilaan ja hoitohenkilöstön vuorovaikutus; Lapsipotilaan ja hoitohenkilökunnan ohjaussuhde; Aktiivinen ja tavoitteellinen toiminta; Ohjattavan asian sisältö) liittyy monia tekijöitä, jotka saattavat vaikeuttaa tai helpottavat potilaan ohjausta.

Tutkimustuloksista ilmeni, että reumasairaan lapsipotilaan ohjaukseen liittyviä asioita ovat iänmukainen ohjaaminen, ohjauksen kertaaminen, ohjaajan kompetenssi, realistisuus ja tuvallisuus tavoitteissa sekä perheen ja yksilöllisyyden huomiointi. Tutkimustuloksista on koottu käsitekartta, jonka tarkoitus on auttaa sairaanhoitajia reumasairaan lapsipotilaan ohjaustilanteissa. Tutkimus tuotti uutta tietoa vähän tutkitusta aihealueesta.

The meaning of this thesis was to illustrate the experiences and sensations about patient guidance of nurses working with children who suffer from juvenile idiopathic arthritis. The goal was to provide in-formation about patient guidance with children suffering from juvenile idiopathic arthritis (JIA), and with the help of that information, to improve it.

The research method was qualitative. The inductive material for the research was collected with the help of a theme interview. The themes and questions used in the interview were created with the help of already existing information about patient guidance. Three nurses who work with children suffering from JIA took part in the interviews. The interviews were carried out during October 2012. The analysis of the material was done using content analysis.

There were six themes in the interview: The context of a child patient and nursing staff; Cognitive devel-opment of children; Interaction between a child patient and a nurse; The guidance relationship between a child and a nurse; Active and meaningful action; The content of the guidance. According to the results of this research all themes include many different factors that can either ease or make the patient guidance more difficult.

It came up from the research that the factors that are involved in patient guidance with children with JIA are guidance according to the age of the child, repetition of the guidance, competence of the guiding nurse, reality and safety in guidance and also individuality and paying attention to the family. The results have been put together in a mind map, the aim of which is to help nurses in guiding situations with children with JIA. The research also provided new information about the field that has been studied quite a little earlier.
