Opas hoito- ja palvelusopimuksen laatimiseen Imatran tehostettuun palveluasumiseen

Abstract

Tämän opinnäytetyön aiheena on hoito- ja palvelusopimuksen laatiminen tehostetussa palveluasumisessa. Opinnäytetyön tuotoksena on laadittu Opas hoito- ja palvelusopimuksen laatimiseen Imatran kaupungin tehostettuun palveluasumiseen. Tehostetussa palveluasuminen on tarkoitettu ikääntyneille, yli 65-vuotiaille asiakkaille, jotka eivät pärjää enää kotona omaisen ja/tai ammattilaistenkaan avun turvin. Palveluasumisen yksiköt, arkikielellä ryhmäkodit, tukevat asiakkaan omatoimisuutta asiakaslähtöisesti ja yksilöllisyyttä kunnioittaen. Jokaiselle ryhmäkodin asiakkaalle tulee laatia hoito- ja palvelusopimus, joka on kooste hänen voimavaroistaan, hoitotyön tavoitteista sekä auttamismenetelmistä. Hoito- ja palvelusopimukseen kirjataan asiakkaan tarvitsemat palvelut.

Opas on hyvin tarpeellinen Imatran tehostetulle palveluasumiselle. Se ohjaa työntekijää hoito- ja palvelusopimuksen teossa; tiedonkeruussa, sopimisessa sekä kirjaamisessa. Opas laadittiin yhteistyössä Imatran tehostetun palveluasumisen johtajan, työelämän ohjaajan sekä muiden sairaanhoitajien kanssa. Keskustellen edellä mainittujen kanssa sekä tutustuen hoito- ja palvelusopimuksiin laadittiin ensimmäinen versio. Sitä käsiteltiin sairaanhoitajien kokouksessa. Toista versiota testasi käytännössä kaksi sairaanhoitajaa ja sen jälkeen siihen tehtiin vielä korjauksia. Opas on helppolukuinen ja otettu käyttöön tehostetun palveluasumisen yksiköissä. Opiskelija on ollut vastuussa oppaan kirjoittamisesta ja valmistumisesta, mutta oppaan päivitys jää tehostetun palveluasumisen vastuulle.

The subject of this thesis is composing a health and service agreement for elderly care in the home. The product of this thesis is a guide about that subject for City of Imatra. Residence homes for the elderly in Imatra are meant for those over 65 who are not able to get along with daily activities at home despite help from relatives and/or professionals. These so-called group homes support the independence of the resident with patient oriented and individualized care. Every resident is able to have preferences for the health and service agreement which is a summary of their resources and goals and required methods of nursing. Services that the resident needs are supposed to be documented in the agreement.

This guide is very important to the homes for the elderly in Imatra. It will help employees with making the agreement: with data collection, discussion and documentation. The guide was written in co-operation with the leader of Imatra’s homes for the elderly, and with the help of other nurses working in the environments. Conversing with them and becoming familiar with the existing agreements aided in the production of the first version of the guide. Theguide was discussed during a meeting of the nurses. Then the second version of the guide was tested in practice by two nurses. After that, more corrections were made. This guide is easy-to-understand and they are now using it in the care homes. The responsibility of writing and completing the guide was the student’s, but the updating will be the responsibility of the staff of the homes for the elderly.
