Kirjallinen ohjeistus potilaan kuljettamattajättämistilanteisiin ensihoitohenkilöstölle

Abstract

Tämän opinnäytetyön tarkoitus oli tuottaa ohjeistus Etelä-Karjalan pelastuslaitoksen Lappeenrannan toimipisteen ensihoitajille potilaan kuljettamatta jättämistilanteisiin liittyen. Tavoitteena oli tehdä toimiva ja selkeä ohjeistus tilanteisiin, joissa ambulanssin hälyttäminen ei syystä tai toisesta johda potilaan kuljetukseen. Ohjeistus laadittiin yhteistyössäensihoidon vastuulääkärin sekä pelastus-laitoksen sairaankuljetuspäällikön kanssa.

Opinnäytetyö koostuu raportti-osasta ja ohjeistuksesta Etelä-Karjalan pelastuslaitoksen, Lappeenrannan toimipisteen ensihoitajille. Raporttiosassa käsiteltiin ensihoidon kehitystä, tehtävämäärien kasvua sekä lakeja, jotka ohjaavat ensihoitajaa päätöksenteossa. Raportissa käytiin läpi myös elvyttämättä jättämisen etiikkaa ja sydänpysähdyspotilaan ennustetta.

Väestön ikääntyessä ensihoidon tehtävämäärät ovat kasvussa. Ensihoidon ja koulutuksen kehittyessä potilaat hoidetaan entistä useammin kotona eikä kuljetusta hoitolaitokseen tarvita. Kynnys avun hälyttämiseen on madaltunut minkä vuoksi ensi- hoitoa kohteessa tai kuljetusta hoitopaikkaan ei aina tarvita.

Etelä-Karjalan pelastuslaitoksen Lappeenrannan toimipisteen alueella vuosittaiset tehtävämäärät kasvavat 8 - 10 prosenttia vuodessa. Ensihoitoyksikön antamaan X-koodiin päättyy noin 50 prosenttia tehtävistä.

Opinnäytetyön kohderyhmänä olivat ensihoitajat ja ensihoitoa opiskelevat. Opinnäytetyönä tuotettu ohjeistus on tarkoitus ottaa käyttöön Etelä-Karjalan pelastuslaitoksen Lappeenrannan toimipisteessä.

The purpose of this thesis was to produce written instructions for the South Carelia Rescue Department`s ambulance employees in Lappeenranta for patients who were assessed and treated, but did not require transportation to a facility for continued care. The goal was to create a workable and clear set of instructions that were written in cooperation with the emergency care physician and the chief of the paramedic staff.

The thesis consist of a report regarding theory and the essential instructions. The report handled theory regarding the development of emergency care, the increased numbers of emergency responses, and also the laws that lead every paramedic in their decision-making. The report also discusss the ethics of ”do not resuscitate” situations as well and the prognosis of a cardiac arrest patient.

The population is aging and that increases the number of paramedic response calls. Emergency care and education have developed so that more and more often the patients are evaluated and treated in their own homes, and there is no need to transport them to a hospital. The threshold for calling 112 is also getting lower which very often means that there is no need to transport to a hospital or even give any emergency care at the scene.

In the area of South Carelia`s Rescue Department in Lappeenranta, the response calls increased have increased by a 8 to 10 percent a year. The number of times when the emergency unit ends their calls for an x-code is approximately 50 percent of all responses.

The target groups of this thesis are all paramedics and paramedic nursing students. The instructions will be put into use in the South Carelia Rescue Department in Lappeenranta.
