Lasten anestesiahoitotyö : Verkko-oppimismateriaalia Tampereen ammattikorkeakoulun lasten ja nuorten hoitotyön vaihtoehtoisten ammattiopintojen opiskelijoille

Abstract

Tämän tuotokseen painottuvan opinnäytetyön tarkoituksena oli tuottaa itsenäisesti opiskeltavaa verkko-oppimismateriaalia lasten anestesiahoitotyöstä Tampereen ammattikorkeakoulun lasten ja nuorten hoitotyön vaihtoehtoisten ammattiopintojen opiskelijoille. Opinnäytetyöstä syntyneen verkko-oppimismateriaalin avulla pyritään palauttaa mieleen perioperatiivisen opintojakson asioita sekä syventää opiskelijoiden tietoa eri-ikäisten lasten anestesiahoitotyöstä.

Lasten anestesiahoitotyö tuo hoitavalle henkilökunnalle omat haasteensa fysiologisten ja anatomisten erojensa vuoksi. Verraten aikuisiin, lasten keuhkot, hengitys, verenkierto, lämpötalous ja aineenvaihdunta ovat huomattavasti erilaisia. (Meretoja 2006, 430.) Tämän vuoksi onkin tärkeää tietää nämä eroavat ominaisuudet ja pystyä ymmärtämään niiden aiheuttamat vaikutukset hoitotyössä.

Opinnäytetyön raporttiosuudessa käsitellyt asiat ovat tiivistettynä versiona tuotoksessa eli itsenäisesti opiskeltavassa verkko-oppimismateriaalissa. Raporttiosuudessa käsittelemme anestesiahoitotyötä keskittyen intraoperatiiviseen vaiheeseen; anestesiamenetelmiin ja – aineisiin, anestesian aikaiseen tarkkailuun ja lasten tuomiin erityispiirteisiin anestesiahoitotyölle. Raporttiosuutemme teksteissä olemme käsitelleet myös pre- ja postoperatiivisen vaiheen hoitoa, sillä kokonaisuuden hahmottamisen kannalta olemme kokeneet sen tärkeäksi.

Olemme poimineet tuotokseen tärkeimmät asiat eri-ikäisten lasten anestesiahoitotyöstä ja niiden vaikutuksista osastolla tapahtuvaan hoitotyöhön. Tuotoksesta muotoutui todellisiin tilanteisiin verrattavissa olevia vuoropuheluja, joissa sairaanhoitaja, lapsi sekä vanhemmat keskustelevat anestesioista ja niihin liittyvistä, huomioon otettavista asioista. Näiden vuoropuheluiden avulla opiskelijan on helpompi omaksua tietoa ja sitä, kuinka se tulisi esittää. Erityistilanteiksi valikoimme vanhemmille anestesiasta kertomisen, leikki-ikäisen lapsen pelon, kehitysvammaisuuden sekä diabeteksen vaikutusta anestesiaan.

Kehitysehdotuksiksi nousivat meidän työstämme poisrajattu keskosen anestesia sekä perioperatiivisen hoitotyön vaihtoehtoisten ammattiopintojen opiskelijoille tuotettu opintomateriaali eri-ikäisten lasten erityispiirteistä. Näin ollen lasten kokonaisvaltainen hoito paranisi, kun lapsia ja esimerkiksi heidän pelkojaan ymmärrettäisiin paremmin.

The aim of this output-oriented thesis was to produce e-learning study material on anesthesiology nursing for the students of child and youth nursing at Tampere University of Applied Sciences. The goal of this thesis was, with the help of the e-learning material to bring back to mind the things learned during the perioperative study course and to deepen the students’ knowledge on anesthesiology nursing of children of different ages.

Children’s anesthesiology nursing brings many challenges for nursing personnel due to children’s physiological and anatomical differences. Compared to adults, children’s lungs, breathing, circulation, body warmth and metabolism are noticeably different (Meretoja 2006, 430). This is why it is important to know of these differences and be able to understand how they affect the nursing.

The subjects discussed in the report section of the thesis are presented in a summarized form in the e-learning material. The report section deals with / discusses anesthesiology nursing focusing on the intra-operative stage; methods and drugs used in anesthesia, monitoring during anesthesia and the specific characteristics children bring to anesthesiology nursing. Pre- and post-operative care are also discussed in the report section.

The most important issues of anesthesiology nursing with children of different ages and their effects on the nursing work at the ward are presented in the learning material. The material includes life-like dialogues in which a nurse, child and parents talk about anes-thesia and the issues that are related to it and must be taken into consideration. With the help of these dialogues it is easier for the students to absorb the information and to learn how it should be presented to clients / patients and their relatives.

It is proposed that further studies should be conducted on the anesthesia of preterm infants, as well as e-learning material on the special features of children of different ages for students of perioperative nursing. When children and for example their fears are understood better, the quality of nursing will improve.
