Angiografiaan ja pallolaajennukseen menevän potilaan ohjaus

Abstract

Opinnäytetyön tarkoituksena oli kuvata Satakunnan keskussairaalan sisätautiosaston KM4:n angiografia- ja pallolaajennuspotilaiden mielipiteitä saamastaan ohjauksesta. Tutkimusjoukkoa rajattiin sepelvaltimotautia sairastaviin potilaisiin, jotka olivat suunnitellusti menossa angiografiaan ja mahdollisesti pallolaajennukseen. Tavoitteena oli saada tietoa potilasohjauksen toimivuudesta ja yleisesti selvittää potilaiden mielipiteitä saamastaan ohjauksesta. Tavoitteena oli myös laatia saatujen tutkimustulosten pohjalta osaston henkilökunnalle minimikriteerit, joiden tulisi täyttyä potilaiden ohjaustilanteessa. Osastolla toteutuvaa ohjausta tarkasteltiin kolmessa eri vaiheessa, joita olivat ohjaus osastolle saapuessa, ennen toimenpidettä ja potilaan kotiutuessa. Aineisto kerättiin strukturoituja, puolistrukturoituja ja avoimia kysymyksiä sisältävällä kyselylomakkeella, johon vastasi 24 osaston KM4 potilasta. Potilaat saivat kotiutuessaan kyselylomakkeet mukaansa ja he postittivat vastaukset osastolle. Vastausprosentti oli 63. Opinnäytetyö oli määrällinen eli kvantitatiivinen. Tutkimusaineisto analysoitiin tilastollisesti. Opinnäytetyössä saatujen tutkimustulosten mukaan ohjaus toteutui yleisesti ottaen hyvin. Ohjauksen eri toteutumisvaiheista potilaat olivat tyytyväisempiä ennen toimenpidettä saamaansa ohjaukseen. Muissa vaiheissa muutama potilas jäi kaipaamaan enemmän ohjaukselta. Vastanneista lähes kaikki olivat saaneet ohjauksesta itselleen tärkeimmiksi kokemansa tiedot ja ohjaustilanteesta oli jäänyt positiivinen vaikutelma. Vastaajista puolet koki ohjaustilanteen tapahtuneen rauhallisessa ympäristössä, kun taas yhden viidesosan mielestä ohjaustilanne ei ollut tarpeeksi rauhallinen. Tutkimus antaa tietoa ohjauksen toteutumisesta käytännössä ja tuottaa tietoa, jonka avulla sisätautiosastolla KM4 ohjaus voidaan pitää mahdollisimman korkealaatuisena. Jatkotutkimuksin voisi selvittää esimerkiksi sydäntoimenpideyksikön ohjauksen toteutumista sekä ohjauksen toteutumista sitä antavien hoitajien näkökulmasta.

The aim of the master’s thesis was to describe what angiography and PTCA patients being treated on the internal disease ward KM4 of Satakunta Central Hospital thought about the guidance they had received. The target group was restricted to include patients with coronary disease who had been referred to angiography and, possibly, to PTCA. The goal was to obtain information on how the guidance of patients worked and, in general, to learn patients’ views on the guidance they had received as well as to, based on the research results, draw up minimum criteria for the ward staff which should be met in guiding patients. The guidance provided on the ward was looked at in three stages: guidance on arrival of the patient, guidance before the intervention and guidance when the patient is discharged from the hospital. The material was gathered by means of a questionnaire including structured, semi-structured and open questions. 24 patients from ward KM4 responded to the inquiry. The questionnaires were given to the patients when they were being discharged, and they posted their responses to the ward. The response percentage was 63. The thesis was quantitative, and the research material was analyzed statistically. The results of the thesis showed that, in general, the guidance was a success. Of the different stages of the guidance, patients were the most satisfied with the guidance provided before the intervention. In the other stages, a couple of patients would have wanted more from the guidance. The guidance had given almost all of the patients the information they themselves had felt was of primary importance, and the guidance itself had made a positive impression on them. One half of the respondents felt the guidance had taken place in a calm environment, whereas one fifth did not consider the guidance situation peaceful enough. The study yields information on the practical implementation of guidance and on how guidance on the internal disease ward KM4 can be maintained at a highest possible level. In future, for example, the practical implementation of guidance provided by the Cardiology Intervention Unit and the implementation of guidance from the point of view of nurses involved in it could be studied.
