HOITOTYÖN TARKISTUSLISTAN LAATIMINEN AIKUISPOTILAAN LAADUKKAAN KOTIUTUMISEN VARMISTAMISEKSI

Abstract

Opinnäytetyö toteutettiin projektimaisena opinnäytetyönä. Tarkoituksena oli suunnitella ja laatia hoitotyön tarkistuslista kotiutuvalle potilaalle laadukkaan kotiutumisen turvaamiseksi. Tarkoituksena oli laatia tarkistuslista kotiutuvalle potilaalle, jossa on tiivistetyssä muodossa kaikki kotiuttamisessa huomioon otettavat asiat. Projektityön tavoitteena oli edistää kotiutuvan potilaan hyvinvointia, turvata kotiutuvan potilaan hoidon jatkuvuus ja lisätä tietoa kotiuttamisessa huomioon otettavista asioista.

Opinnäytetyössä kotiutumista käsiteltiin potilaan, hoitohenkilökunnan ja kotihoidon henkilöstön näkökulmasta. Näyttöön perustuvaan teoriatiedon perusteella laadittiin kotiutuvan potilaan tarkistuslista, johon on koottu asioita, jotka tulee tehdä kotiutta-essa potilasta. Käytännön kokemukset huomioitiin myös tarkistuslistaa laatiessa.

Tarkistuslista on käyttökelpoinen kotiuttaessa potilasta joko kotiin tai jatkohoito-paikkaan. Hoitaja voi tarkistaa listalta, että on muistanut ottaa huomioon kotiuttamiseen liittyvät asiat. Hoitaja merkkaa jokaisen tarkistettavan kohdan jälkeen joko ”kyllä tai ”ei” sen mukaan, onko kyseinen asia tehty vai ei. Hoitajalla on tilaa kommenteille jokaisen tarkistettavan kohdan jälkeen olevalle ”muuta huomioitavaa” sarakkeelle. Tarkistuslista on kolmen A4-sivun pituinen ja siinä on 37 tarkistettavaa kohtaa.

Tarkistuslista otetaan käyttöön opinnäytetyön tekijän toimesta sairaalan vuodeosastolla. Tarkistuslista on kuitenkin muokattavissa kunkin osaston tarpeisiin.

This thesis was made as a project. The purpose of this thesis was to design and make a nursing checklist for patient who is being discharged from the hospital to ensure his/her safe discharge. The purpose of this thesis was to make a checklist that is a summary of issues related to discharging a patient. The purpose of this project was to promote the wellbeing of the discharged patient, ensure the continuity of the caring of the discharged patient and increase the information of discharging a patient.

In this thesis the concept of discharging a patient has been dealt with the patient, nursing staff and the staff of the community point of view. The checklist has been made based on the evidence-based information of discharging a patient. The check-list has things which are need to be done when discharging a patient. The experiences from the practise have also been taken into consideration when making the checklist.

The checklist is useful when discharging patient either home or to place of continued care. The nurse can check from the list whether he or she has remembered to do everything when being discharging a patient. The nurse will mark “yes” or “no” accord-ing to if thing has been made or not. In the checklist after every point there is room for nurse’s comment. The checklist has three A4-pages and it has 37 points that need to be checked.

The checklist will be brought into use in a hospital ward by the maker of this thesis. The checklist is customizable into needs of different wards however.
