OYS ERVA – alueen keskussairaaloista tapahtuvien tehopotilaiden sekundaarisiirtojen laatu siirtokuljetusdokumenttien kirjausten perusteella arvioituna

Abstract

Oulun yliopistollisen sairaalan erityisvastuualueen (OYS ERVA) sairaanhoitopiirit ovat Lapin, Länsi-Pohjan, Kainuun ja Keski-Pohjanmaan sairaanhoitopiirit. OYS ERVA -alueella on omat tehopotilaan sairaalasiirto – ohjeet, eli sekundaarisiirtojen ohjeet, jotka on laadittu yhtenäistämään siirtokuljetusten aikaista toimintaa ja takaamaan yhtenäisen hoidon tason sekundaarisiirron aikana. Ohjeistuksen lähtökohtana on, ettei sekundaarisiirron aikana potilaan hoito saa huonontua. Potilassiirtoon osallistuvan henkilökunnan osaamisen taso ja työn laatu sekä ambulanssin työolosuhteet vaikuttavat viimekädessä siirrettävän potilaan hoidon laatuun. Tämän kehittämistehtävän menetelmän ote on yhteistyöllinen. Kehittämistehtävän tarkoituksena oli kartoittaa kvantitatiivisen tutkimuksen avulla OYS ERVA – alueen keskussairaaloiden sekundaarisiirtojen laatu kirjausten perusteella havaintomatriiseja hyödyntäen ja vastata seuraaviin tutkimuskysymyksiin: Minkälainen on tehopotilaan sekundaarisiirron aikainen hoidon laatu siirtodokumenttien kirjausten perusteella arvioituna? Minkälaisia ongelmia havaitaan tehohoitopotilaan hoidossa ilmenneen sekundaarisiirtojen aikana siirtodokumenttien kirjausten perusteella?

Osana kehittämistyötä muodostettiin olemassa olevista mittareista havaintomatriisit, joiden avulla tehohoitopotilaan siirtokuljetusten aikaista kirjattua hoidon laatua voitiin arvioida kvantitatiivisesti. Kvantitatiivisen tutkimuksen aineistona olivat tehopotilaiden ulos- ja sisäänkirjausraportit, sekundaarisiirron dokumentaatiot eli siirtokaavakkeet (liite 1), sekä Therapeutic Intervention Scoring System (TISS) ja sequential organ failure assessment (SOFA) pisteytykset, jotka oli tehty ennen ja jälkeen tehopotilaan sekundaarisiirtoa. Aineisto muodostui kaikista vuosina 2008–2011 tehdyistä tehopotilaiden sekundaarisiirroista, joita oli 79 kappaletta eli kyseessä oli kokonaistutkimus. Aineisto käytiin läpi ja havaintomatriisien avulla kirjallisista dokumenteista poimittiin havainnot tiedonkeruulomakkeelle, tämän jälkeen aineisto taulukoitiin ja syötettiin tilastotieteelliseen analyysiin suunniteltuun ohjelmistoon IBM SPSS Statistics ohjelmalla. Tutkimustulosten mukaan tehohoitopotilaiden sekundaarisiirtojen laatu vaihteli OYS ERVA–alueen keskussairaaloiden välillä, kirjausten perusteella arvioituna niin valvontalaitteiden käytössä kuin tehohoitopotilaan elintoimintojen havainnoinnissa niin sekundaarisiirron alussa kuin lopussa. Tulosten mukaan potilassiirtokaavakkeiden kirjausten perusteella OYS ERVA – alueen sekundaarisiirtoja seurantaväli 62 % seurantaväli oli <30min. Potilaan seurannassa peruselintoimintojen mittaamisessa tuli olla kirjattuna vähintään kolme arvoa: ensiarvio, täsmennetty arvio sekä hoitojen jälkeen kirjatut arvot. Nämä arvot oli tulosten mukaan kirjattu OYS ERVA – alueen keskussairaaloista tapahtuvissa sekundaarisiirroissa 87 %:sti. Saatuja tuloksia voidaan hyödyntää niin OYS:in Teho-osastoilla 1 ja 2 kuin OYS ERVA–alueen keskussairaaloissa tehohoitopotilaan sekundaarisiirtojen ja niiden aikaisten potilaan hoidon kirjaamisen kehittämisessä sekä hoidon että sen kirjaamisen laadunvarmistamisessa.

The special responsibility areas of the Oulu University Hospital (OYS ERVA) hospital districts are the hospital districts of Lapland, Länsi-Pohja, Kainuu and Keski-Pohjanmaa. OYS ERVA-area has its guidelines on the hospital transfers of intensive care patients, in other words guidelines on secondary transfers, which have been agreed to standardize the activities during the transfers and to guarantee equal level of care during secondary transfer. The starting point in the guidelines is that the care of the patient shall not deteriorate during the secondary transfer. The method of this development assignment is cooperative. The purpose of this development assignment is to survey with quantitative research the quality of secondary transfers in the central hospitals of OYS ERVA-area based on records by using observation matrix and to answer the following re-search questions: What is the quality of the care of the intensive care patient during secondary transfer based on evaluation of the records made to the transfer documents? What kind of prob-lems are detected in the care of the intensive care patient having occurred during the secondary transfer based on evaluation of the records made to the transfer documents?

As part of the development assignment I formed observation matrixes based on predefined met-rics, which enabled the quantitative evaluation of quality of the recorded care of intensive care pa-tients during transfers. The data of the quantitative research were the in and out registration re-ports of intensive care patients, documentation of the secondary transfers i.e. transfer forms (ap-pendix 1) and Therapeutic Intervention Scoring System (TISS) and Sequential Organ Failure As-sessment (SOFA) scores, which were given before and after the secondary transfer of the intensive care patient. The data consisted of all secondary transfers of intensive care patients during the years 2008-2011, the amount was 79 pieces and it was a total research. The data was reviewed and using the observation matrixes the observations were picked up from the written documents, which were tabulated and entered in to statistical analysis with IBM SPSS Statistics software. According to the research results the quality of care of intensive care patients during secondary transfers varied between the central hospitals in OYS ERVA-area, based on the evaluation of the records as well as with respect to the use of monitoring devices as with respect to observation of the vital functions of the intensive care patient both at the beginning and at the end of the secondary transfer. According to the results by documentation entries of the secondary transfers follow-up interval was 62% <30 min. Patient monitoring vital signs measurement had to be registered for at least three values and these were recorded by results in 87 % of the secondary transfers. The results received can be used in OYS intensive care units 1 and 2 and in the central hospitals of ERVA-area in improving the recording of the care of the patient and in quality control of the care and its recording of secondary transfers of intensive care patients.
