CRP:n vieritutkimus ja hoitopäätöksen tekeminen : kyselytutkimus Turun kaupungin terveysasemien hoitohenkilökunnalle

Abstract

Opinnäytetyö oli osa VIENO -hanketta, joka on Turun ammattikorkeakoulun ja Turun kaupungin hyvinvointitoimialan yhteistyöprojekti. Sen tavoitteena oli kehittää vierinäytteenottotaitoja. Opin-näytetyön tarkoituksena oli selvittää Turun kaupungin terveysasemien käytäntöjä C-reaktiivisen proteiinin vierinäytteenotossa ja tavoitteena oli tutkia, miten CRP -vierinäytteiden hyödynnetään hoitotyössä. Tutkimus oli pääosin määrällinen survey-tutkimus. Tutkimuksen kohteena olivat Turun kaupungin suomenkieliset terveysasemat. Tutkimusmenetelmänä oli kysely, joka lähetettiin kaikille terveysasemilla sillä hetkellä työskenteleville hoitajille sähköisen Webropol® -ohjelman avulla. Aineisto kerättiin ja osittain analysointiin Webropol® -ohjelman avulla. Kuviot ja taulukot tehtiin Microsoft Exel- ohjelman avulla. Kyselyn avulla pyrittiin selvittämään hoitajien valmiuksia CRP -vierinäytteenottoon, sekä tulosten hyödynnettävyyteen hoitotyössä. Kyselyn avulla tutkittiin myös Turun kaupungin yhtenäisiä käytäntöjä CRP- vierinäytteenotosta sekä tulosten kirjaamisesta. Kysely lähetettiin 91 hoitajalle, joista 17 vastasi. Vastausprosentti oli noin 18,7. Tutkimuksen avulla saaduista tuloksista selvisi, että Turun kaupungin hoitajat eivät juuri tee itsenäisiä päätöksiä CRP -vierinäytteenotosta tai sen hyödynnettävyydestä hoitotyössä. Hoitajat ottavat CRP -vierinäytteen lähinnä lääkärin määräyksestä. Pääsääntöisesti hoitajat suhtautuivat CRP -vierinäytteenottoon myönteisesti ja kokivat sen osaksi hoitoa. Vierinäytteiden kirjaamiseen ei löytynyt yhteistä käytäntöä, vaan kaupungin alueella kirjaamistavat saattoivat vaihdella huomattavasti.

The thesis was part of VIENO -project, which is collaborative project between Turku University of Applied Sciences and The City of Turku´s Welfare Division. The aim of the project was to improve the skills of point-of-care testing (POCT). The aim of this study was to clarify health care centers of Turku practices in POCT of CRP. The purpose of the study was to research, how the POCT of CRP is utilized in nursing. The study was mainly quantitative survey study. Target of the study was Finnish health centers of Turku. The study was done by using Webropol® Internet program to send the questions. The material were collected and partly analyzed with Webropol® program. Figures and tables were made with Microsoft Exel- program. The attempt of the study was to clarify, how nurses relate to the POCT of CRP and how they utilized it. The study also tried to find, how coherent the health care centers are doing the POCT and documenting the data. Questionnaires were sent to 91 nurses, of whom 17 responded. The response rate was 18.7 The results that were found out were that the nurses didn’t make the decision of the care of POCT of CRP. The nurses didn´t use POC- testing much in their work. Mostly nurses took POCT of CRP because a practitioner had ordered it. Nurses reacted positively in POCT. There were uniform standard in documenting the data and the recognition could vary within the city.
