Omaolo Kittilään: kansallisen digipalvelun käyttöönottoprosessi

Abstract

Tämän opinnäytetyö on tehty Kittilän kunnan terveyskeskuksen tilauksesta Omaolon oirearvioiden käyttöönotosta Kittilässä. Omaolo oirearviot ovat valtion omistaman erityistehtäväyhtiön DigiFinlandin tuottama kansallinen digitaalinen palvelu. Omaolon oirearvioiden osio on lääkinnällinen laite, joka antaa oireisiin sopivat ja lääketieteelliseen tutkimustietoon perustuvat arviot hoidon tarpeesta ja kiireellisyydestä tai kotihoito-ohjeet.

Tämän opinnäytetyön tavoitteena oli tuoda päiväkirjamuodossa, tapauskohtaisesti esille kansallisen digitaalisen palvelun oirearvioiden käyttöönottoprosessi pääkäyttäjän näkökulmasta. Opinnäytetyön tarkoituksena oli luoda Kittiläläisille oma Omaolo, jonka paikalliset ohjaukset ja ammattilaisten jonotukset vastaavat Kittiläläisten ja Kittilässä matkailevien sekä terveyskeskuksen henkilökunnan tarpeisiin. Tarkoituksena oli myös mitata Omaolon oirearvioiden tuottavuutta ajallisesti Kittilän terveyskeskuksen päivystyksessä sekä seurata Omaolon oirearvioiden asukkaiden käyttömäärää käyttöönotosta kesäkuun loppuun. Opinnäytetyössä tutkimuskysymyksenä on kuinka tuoda digitaalinen ohjelma Lappilaiseen terveyskeskukseen, kerrotaan mitä tehtiin ja lopuksi mitä olisi pitänyt tehdä toisin.

Opinnäytetyöstä Omaoloa käyttöönottava uusi kunta saa vinkkiä markkinoinnista, ammattilaisten mukaan ottamisesta sekä digitalisaation johtamisesta. Opinnäytetyön tuloksista voidaan mainita oirearvioiden vaikuttavuudesta esimerkiksi virtsatieinfektion hoidosta ammattilaisen ajankäytön lyhenevän useilla minuuteilla, kun verrataan puhelimessa tehtävää hoidontarpeen arviota.

Omaolon saapuminen Kittilään linkittyi vahvasti sosiaali- ja terveysalan sekä pelastustoimen uudistukseen. Tässä opinnäytetyössä on kuvattu sosiaali- ja terveysalan sekä pelastustoimen uudistusta digitaalisuuden näkökulmasta, lisäksi on tuotu esille digitaalisen välineen tuomista uuteen ympäristöön sekä johtamisen näkökulmasta, että ammattilaisen näkökulmasta. Tietoperustaan on sisällytetty kuvaus Kittilän kunnasta sekä DigiFinlandista ja Omaolosta itsestään. Opinnäytetyö perustuu kirjoittajan muistiinpanoihin Kittilän Omaolon pääkäyttäjänä.

This thesis was commissioned by the Kittilä municipality health center on the launching and implementation of Omaolo symptom checkers in Kittilä. Omaolo symptom checkers are a national digital service produced by a state-owned specialist company DigiFinland. Omaolo symptom checkers section is a service activity that provides assessments of the need and urgency of treatment or home care instructions that are appropriate for the symptoms based on medical research data.

The aim of this thesis was to introduce the implementation process of symptom checkers of the national digital service in a diary form, on a case-by-case basis, from the administrator point of view. The purpose of the thesis was to create an Omaolo for the residents of Kittilä municipality to meet the needs of the residents and the people traveling in Kittilä, as well as the health center staff. The purpose was also to measure the productivity of the Omaolo symptom checkers over time in the Kittilä health centrer emergency room, and to monitor the number of residents usage of Omaolo symptom checkers during the period from commissioning to July 15, 2022. The research question of this study addressed the mans on how to bring a digital program to a health center located in Finnish Lapland and to report what was done and what should have been done differently.

Thesis theoretical database of this study includes a description of Kittilä municipality as well as DigiFinland and Omaolo. The theoretical information is also based on the author's notes as the administrator of the Kittilä Omaolo service. The arrival of Omaolo in Kittilä was linked to the reform of the social and health sector and rescue operations in Finland. This thesis focused on the reform from a digital point of view. The introduction of a digital tool into a new environment was addressed from the point of view of management and from the point of view of health care professionals.

This thesis can potentially help municipalities implementing Omaolo with tips on marketing, involving professionals, and managing digitization. The results of the thesis showed that e.g., for the treatment of a urinary tract infection the time of the professionals used in the cases is reduced by several minutes, when compared to the needs assessment for treatment made over the phone.
