Ikääntyneen laitospäihdekuntoutus: asiakkaiden ja henkilökunnan kokemuksia

Abstract

Tässä opinnäytetyössä teoreettinen näkökulma painottuu ikääntyvän päihdeongelmaisen laitospäihdekuntoutukseen. Tarkoituksena on selvittää kuntoutumista ja toipumista edistäviä toimintatapoja ja saada asiakasnäkökulmaa kuntoutumisesta. Tavoitteena on tuottaa tietoa päihdehoitolaitoksen henkilökunnalle siitä, miten ikääntyvän kuntoutusprosessia tulisi tukea ja kehittää. Tutkimuskysymyksinä ovat: Mitkä asiat ikääntyvä asiakas kokee merkitykselliseksi kuntoutumisessaan ja toipumisessaan? Miten ikääntyvien kuntoutusta pitää kehittää laitoksessa?

Päihdehoitolaitoksessa pääpaino on päihdeongelmasta toipumisessa. Ikääntyneet ovat kuitenkin kasvava joukko kuntoutuspalveluiden piirissä olevia, ja heidän kuntoutuksensa kehittäminen on tulevaisuudessa tärkeä tehtävä. Opinnäytetyö on laadullinen opinnäyte työ. Aineisto kerättiin kahdessa osassa kevään ja kesän 2022 aikana.

Asiakasnäkökulmaa varten kerättiin aineisto teemahaastatteluina. Asiakkaan omakohtainen kokemuksellisuus kuntoutuksen hyödyistä ja haasteista oli pohjana kehittämisnäkökulmalle. Haastattelut analysoitiin sisällönanalyysin keinoin. Toisessa vaiheessa henkilökunta innovoi omia ehdotuksiaan kehittämistyöhön yhteiskehittämispajoissa. Pajoissa hyödyn nettiin asiakkaiden esiin nostamia teemoja. Haastattelujen ja kehittämispajojen pohjalta opinnäytetyö tuo esiin kehittämisehdotuksia kuntoutuksen kehittämiseksi.

Asiakkaiden haastattelujen pohjalta nousi esiin kolme pääluokkaa: asiakkaan saama päihdekuntoutus, asiakkaan saama tuki henkilökunnalta ja asiakkaan saama tuki samanikäisiltä asiakkailta. Tuloksissa painottui samanikäisyyden merkitys vertaistuen saamisessa. Henkilökunnan ehdotuksissa korostuivat työn kehittäminen, osaamisen ja kuntoutuksen sisällön kehittäminen sekä ympäristön mahdollisuuksien ja haasteiden huomioiminen. Huoli ikääntyvän päihdeongelmaisen asiakkaan pärjäämisestä tulevaisuudessa on ilmiö, joka koskettaa monia rajapintoja.

Laitoskuntoutuksessa ikääntyneiden asiakassegmentti on melko marginaalinen, mutta kasvava ryhmä. Hoidon laadun ja osaamisen kehittämisen näkökulmasta ikääntyneiden parissa työskentelevien substanssiosaaminen on erityisosaamista, jota tulevaisuudessa on syytä kehittää. Aiheen merkityksellisyyden näkökulmasta kaikki tässäkin opinnäytetyössä käyty keskustelu, kehittäminen ja asiakasnäkökulman kerääminen edistävät ikääntyvien päihdekuntoutusta. Työn hyöty näkyy ensisijaisesti laitospäihdekuntoutuksessa.

The theoretic point of view in this thesis is on the in-patient rehabilitation of an elderly sub stance addict. The point is to examine the ways of acting that advance rehabilitation and recovery as well as get the clients’ point of view of rehabilitation. The goal is to produce information on how to support and develop rehabilitation of an elderly patient for the staff of an in-patient addiction rehabilitation center. We focus on the following questions: What aspects does an elderly client find meaningful in their rehabilitation and recovery? How do we need to develop elderly rehabilitation at the rehabilitation center?

In an addiction rehabilitation center, the main focus is on recovering from an addiction of substances. The elderly are an ever increasing group within rehabilitation services, and thus developing their rehabilitation will be an important mission in the future. This thesis is a qualitative study. The study material was gathered in two parts. The material for the clients’ point of view was gathered via a themed interview. The base for the development point of view was in the clients’ personal experiences of the benefits and challenges they had faced. The interviews were analyzed by means of content analysis. Afterwards the staff processed the clients’ experiences and innovated their own ideas for developmental work in co-development workshops. Based on the interviews and workshops, this thesis brings up suggestions for development to be used by the work community.

Based on the interviews conducted with the clients, three main themes were prominent: the rehabilitation the client had received, support from the staff that the client had experienced, and support that the customer got from other clients similar to their age. When reflecting on their recovery, the significance of other clients in the same age group was emphasized in the results. In the staff’s suggestions, developing the profession, know-how and rehabilitation as well as utilizing the environment more effectively were emphasized. In the future, concerns about how the elderly addiction rehabilitants will fare, will be a phenomenon that will be relevant in many interfaces.

In the world of in-patient rehabilitation, the client segment of the elderly is still a marginal, but an increasing group. From a care quality and know-how developmental point of view, the substance knowledge of the professionals working with elderly clients is special expertise and should be developed in the future. Looking at this theme from its significance perspective, all the conversations, development and gathering of client experiences will advance the rehabilitation of elderly substance addicts. The benefits of this work will be most visible in in-patient rehabilitation.
