Potilasturvallinen rintakipupotilaan hoitotyö vuodeosastolla – EKG-toimenpideosaamista kehittämällä

Abstract

Sosiaali- ja terveyspalveluiden yksiköissä pyrkimyksenä on pitää yllä riittävää osaamista jatkuvan työn kehittämi-sen avulla. Osaamisen kehittämistä tapahtuu työntekijätasolla erilaisten kehittämiskeinojen avulla. Hoitajan kliini-set taidot sekä opetusohjausosaaminen korostuvat hoitotyössä. Potilasturvallisuus on huomioitava koko hoito-prosessin ajan, johon kuuluvat laite ja lääketurvallisuus. Opinnäytetyön aiheena oli rintakipupotilaan hoitotyön kehittäminen EKG-toimenpide osaamisen avulla. Vuode-osastolla hoitohenkilökunnan tulee tunnistaa rintakipupotilaan oireet ja potilasta tulee hoitaa asiakaslähtöisesti. Opinnäytetyön tavoitteena on kuvata hoitohenkilökunnan potilasturvallista rintakipupotilaan hoitotyötä ja tarkoi-tuksena on vuodeosaston hoitohenkilökunnan potilasturvallisen rintakipupotilaan hoitotyön kehittäminen EKG-toimenpideosaamista kehittämällä. EKG-tutkimusmenetelmänä on merkityksellinen potilaan sydäninfarkti epäilyssä. Tutkimus tulee suorittaa vir-heettömästi. Ekg: n avulla kuvataan sydämen sähköistä toimintaa, jonka avulla arvioidaan sydänlihaksessa tapah-tuvia mahdollisia muutoksia. Potilaan iholle laitettujen elektrodien ja johtimien avulla tarkastellaan elektrodien välisiä potentiaalieroja, joita mitataan ajan funktioina. Toimenpiteen onnistumisen edellytyksenä on hoitajan riit-tävä osaaminen tutkimuksen suorittamiseen. Potilas valmistellaan toimenpiteeseen asianmukaisesti ja hoitajan tulee hallita Ekg-laitteen käyttö ohjeiden mukaisesti. Opinnäytetyöprosessissa kehitettiin posteri toimintaohjeeksi vuodeosaston hoitohenkilökunnan käyttöön, jonka avulla lisätään potilasturvallista rintakipupotilaan hoitotyötä vuodeosastolla ja yhtenäistetään hoitohenkilökunnan EKG- toimenpiteeseen liittyviä käytänteitä.

Maintaining and developing sufficient professional competence is important in social and health car. Different measures are employed to develop social and health care professionals’ competence. Good practical skills as well as teaching and counselling competence are highlighted in nursing. Patient safety that also includes equipment and drug safety is considered throughout the nursing process. The subject of this thesis is the development of nursing care of patients with chest pains through EKG skills. In an inpatient ward it is crucial that the staff recognize a patient's symptoms and provide appro-priate care to the patient with sufficient professional skills. The aim of this thesis was to develop the professional competence of the staff at Taivalkoski Health Center to treat patients with chest pains by developing their EKG-skills. The purpose was to describe what kind of competence the ward staff needed to treat patients with chest pains. When suspecting a heart attack, EKG (electrocardiography) is an important examination method that has to be performed correctly. An EKG-describes the electrical activity of the heart, which helps to assesses possible changes in the heart muscle. Electrodes placed on the patient’s skin help to examine potential differences between the electrodes that are measured as functions of time. To successfully perform an EKG, a nurse has to have sufficient professional competence to prepare the patient for the procedure and to know how to use the EKG-machine properly. This thesis is a functional thesis which consists of two parts (report and product). The report describes some important issues which have to be observed during the whole EKG-procedure. The product is a set of instructions made for the health center staff. The purpose of the instructions is to increase pa-tient safety in the nursing care of patients with chest pains and unify EKG practices.
