Suuhygienistin sateenvarjo : yhteisöllisen suun terveyden edistämisen toimintamalli suuhygienistin työhön Helsingin julkiseen suun terveydenhuoltoon

Abstract

Tässä opinnäytetyössä toteutuu työelämän tarpeiden mukainen kehittäminen, jota ammattikorkeakoululaissa (351/3003) tutkimus- ja kehitystyöltä edellytetään. Tässä opinnäytetyössä oli tarkoituksena suunnitella Helsingin julkiseen suun terveydenhuoltoon suuhygienistille, yhteisölliseen suun terveyden edistämiseen toimintamalli, mikä ohjaa suuhygienistin käytännön työtä, toiminnan prosessissa, alusta loppuun.

Opinnäytetyö, toimintamallin suunnittelu, oli toteutettu keräämällä laaja teoreettinen viitekehys, jossa avattiin työn tärkeimmät käsitteet yhteisöllisen suun terveyden edistämisen ja moni ammatillisen yhteistyön sekä yhteisöjen terveyden lukutaidon tietoperusta. Toteutuksessa teoria ja suun terveydenhuollon nykytilan yhdistäminen, johtavat suuhy-gienistin konkreettisen prosessimallin suunnitteluun. Suuhygienistin toimintamallin kehittäminen lähti tarpeesta yhtenäistää Helsingin julkisen suun terveydenhuollon eri yhteisöjen terveyden edistäminen sekä saattaa se tasaarvoiseksi ja koko elämän kaaren ajan kestäväksi jatkumoksi. Toteutuksessa toimintamallin suunnittelua ohjasi lisäksi Precede – Proceed – yhteisöjen terveyden edistämistä ohjaava malli, koska se oli otettu jo käyttöön terveyden edistämisen ydinprosessin suunnittelussa, Hgin julkisessa suun terveydenhuollossa. Toteutuksen teoreettiseen viitekehykseen sisältyivät myös kyseisen mallin suunnittelun ja arvion tarpeet.

Opinnäytetyön tuloksena syntyi suuhygienistin työhön toimintamalli, yksinkertainen prosessikartta toiminnan eri vaiheista. Opinnäytetyön tuloksena myös kuvattiin toiminnan rakenne sekä sen sisältö elämänkaaren mukaisine, yksityiskohtaisine tarpeineen.

Johtopäätöksenä todetaan, että toimintamalli, joka on nimeltään ”Suuhygienistin Sateenvarjo”, on suuhygienisti-koulutuksen ekspansiivisen oppimisnäkemyksen mukaisen, koulutuksen ja työelämän välisen kehittävän siirtovaikutuksen konkreettinen tuote. ”Suuhygienistin Sateenvarjo” viedään suuhygienistin työtoimintaan Hgin julkiseen suun terveydenhuoltoon, jossa se voi kehittyä vastaamaan yhteisöllisen terveyden edistämisen tarpeisiin. Toimintamallin käyttöä seuraamalla laadullisen tutkimuksen avulla, ensin sitä käyttävän suuhygienistin kokemusten lähtökohdista ja myöhemmin sen käytön vaikuttavuutta seuraamalla, ovat sen jatkuvan kehittymisen ja oikean arvioinnin edellytyksinä.

This diploma project investigates the development needs of oral health promotion, which is required by the legislation of the Polytechnics (351/3003) with respect to research and development. The purpose of this diploma project was to design a model of communal oral health promotion course of action for the Public Oral Healthcare for the City of Helsinki; this guides the practises of the dental hygienist at work from the start to the end.

The diploma project was executed by gathering a wide theoretical frame of reference, where the most important concepts were presented. These concepts were communal oral health promotion and multi-professional co-operation.The starting point for the design was to combine the theory and the present practices in order to produce a concrete course of action for the dental hygienist. The urge to create a course of action for dental hygienists was based on the need to uniform the operations of health promotions for different communities. In addition, further attempts were made to equalize the taken actions and form them into a lifelong procedure. Precede-Proceed -model was used in the study, which is a model for communal health promotion that is already utilized in Helsinki. The study focused on the needs of planning and evaluating this model.

In conclusion, the course of action, called "The Umbrella of the Dental Hygienist" developed as a concrete product of the developing transfer between education and work life. It follows the idea of expansive learning used in the dental hygienists’ education. The Umbrella of the Dental Hygienist is to be implemented in the public oral healthcare in Helsinki, where it may develop to respond to the needs of the communal health promotion. Following the use of this model with means of qualitative research, firstly the experiences of using a dental hygienist and later analyzing its use, will be the prerequisites of the continuous development and the appropriate evaluation of the model.
