EKG:n rekisteröinti akuuttihoitotyössä : Moodle-alusta hoitotyön opiskelijoille

Abstract

Elektrokardiogrammi (EKG) eli toiselta nimeltään sydänfilmi on jo pitkään käytetty tutkimus, jonka avulla voidaan saada tietoa sydämen toiminnasta ja erilaisista sydänsairauksista. Lisäksi se on helppo toimenpiteenä, eikä siitä aiheudu vaaraa potilaalle. EKG perustuu sydämen sähköisen toiminnan mittaamiseen. Sydämen sähköistä toimintaa mitataan ihon päälle asetettavien elektrodien kautta, joiden paikat ovat standardoitu tarkkaan. EKG:n ottaminen oikein on tärkeää, sillä sen avulla saadaan tehtyä diagnoosi sekä tunnistettua mahdolliset rytmihäiriöt. Huonolaatuinen EKG voi nostaa riskiä virheelliseen diagnoosiin tai sen takia voi jäädä jokin rytmihäiriö huomaamatta. Lisäksi oikein otetun EKG:n avulla voidaan huomata sellaiset rytmihäiriöt, joiden hoitaminen vaatii nopeaa reagointia. EKG:n ottaminen kuuluu esimerkiksi akuuttihoitotyössä hoitajan perustehtäviin, jonka vuoksi sen ottaminen ja perustulkinta ovat tärkeitä osata.

Tämä opinnäytetyö toteutettiin toiminnallisena opinnäytetyönä. Teoreettinen tietopohja koostui hoitotieteellisistä tutkimuksista, jotka haettiin ProQuest Central, Cinahl, Medline sekä PuBmed tietokannoista. Hakutulokset rajattiin vuonna 2018–2023 julkaistuihin tutkimuksiin. Hoitotieteellisten tutkimusten lisäksi opinnäytetyössä käytettiin muita terveystieteen julkaisuja.

Opinnäytetyössä tuotettiin verkossa oleva kuvallinen ohje 12-kytkentäiseen EKG-rekisteröintiin, V4R-rekisteröintiin sekä V7-9 rekisteröintiin. Ohje on osa opetusmateriaalia Metropolian ammattikorkeakoulun akuuttihoitotyön opintojaksolla. Ohjeessa on kuvattu luurangon etu- ja takapuoli rintakehän alueelta. Ohjeen kuviin on linkitetty tekstiosioita ja lisäkuvia joissa mm. ohjeistetaan EKG-kytkentöjen laittaminen oikeille kohdille.

Tämän opinnäytetyön tavoitteena on lisätä ja vahvistaa osaamista luotettavan EKG:n ottamiseen ja yleisimpien rytmihäiriöiden tunnistamisessa.

An electrocardiogram (ECG), also known as a heart film, has been used for a long time to obtain information about the heart’s function and various heart diseases. In addition, it is an easy procedure and poses no risk to the patient. The ECG is based on measuring the electrical activity of the heart. The electrical activity of the heart is measured through electrodes placed on the skin, the positions of which are precisely standardized. Taking the ECG correctly is important, as it helps to make a diagnosis and identify possible arrhythmias. A poor-quality ECG can increase the risk of an incorrect diagnosis, or because of it, an arrhythmia may go unnoticed. In addition, with the help of a correctly taken ECG, it is possible to notice such arrhythmias, the treatment of which requires a quick reaction. Taking an ECG, for example, is one of the basic duties of a nurse in acute care work, which is why it is important to know how to take it and its basic interpretation.

This thesis was implemented as a functional thesis. The theoretical knowledge base consisted of medical studies that were searched from the ProQuest Central, Cinahl, Medline and PuBMed databases. The search results were limited to studies published in 2018-2023. In addition to nursing studies, other health science publications were used in the thesis.

The thesis produced online pictorial instructions for 12-lead ECG registration, V4R registration and V7-9 registration. The instruction is part of the teaching material in the acute nursing study course at Metropolia University of Applied Sciences. The instructions describe the front and back of the skeleton from the chest area. Text sections and additional pictures are linked to the pictures in the instructions, where for example instructions are given for putting the ECG connections in the right places.

The aim of this thesis is to increase and strengthen competence in taking a reliable ECG and identifying the most common arrhythmias.
