NIV hoidon toteutus akuutissa hengitysvajaudessa

Abstract

Akuutti hengitysvajaus on yksi yleisimmistä peruselintoimintojen häiriöistä, joka johtaa tehohoitoon tai tehovalvontahoitoon. Hoidon aloituksen viivästyminen lisää kuolleisuuden riskiä. Jokaisen hoitajan tulisi pystyä tunnistamaan peruselintoimintojen häiriö, hälyttää oikea-aikaisesti lisäapua sekä osata henkeä pelastavat toimenpiteet. Hoitotoimenpiteiden sekä laitteiden kehityksen myötä akuuttia hengitysvajautta pystytään hoitamaan tapauskohtaisesti myös ilman invasiivista ventilaatiota. Tutkimukset ovat osoittaneet, että noninvasiivisilla ventilaattoreilla (NIV) on vähemmän haittavaikutuksia ja on yleensä myös halvempi hoitomuoto. Oikealla hoitomuodon valinnalla voidaan lyhentää myös hoitojakson pituutta.

Yksittäisten laite-erojen vuoksi, tässä opinnäytetyössä käsitellään laitteiden yleisimpiä säätöjä. Opinnäytetyön tarkoituksena oli tuottaa opasvihkonen, jota hoitajat voivat hyödyntää NIV-hoidon aloituksessa ja säätöjen asettamisessa. Opinnäytetyön tavoitteena oli lisätä tietoa NIV-hoidosta, yhtenäistää hoidon toteutusta sekä lisätä potilasturvallisuutta. Opinnäytetyön tehtävänä oli selvittää, kuinka akuutin hengitysvajauden hoitoa toteutetaan noninvasiivisella ventilaattorilla.

Opinnäytetyön tuotos, opasvihkonen, sisältää taulukoita ja kuvia säädöistä ja säätöjen merkityksistä. Opasvihkonen on luotu helposti ja nopeasti luettavaksi, jotta sitä voidaan käyttää jopa kesken hoitotoimenpiteiden. Vihkosta voidaan käyttää myös opetuksen tai koulutuksien tukena. Tämän opinnäytetyön tilaaja oli Tampereen ammattikorkeakoulu (TAMK) ja kaikki tämän opinnäytetyön käyttöoikeudet kuuluvat Tampereen ammattikorkeakoululle. Opasvihkosen hyödyllisyyttä ja käytettävyyttä voisi jatkossa tutkia määrällisellä- tai laadullisella tutkimuksella.

Acute respiratory failure is one of the most common basic organ dysfunction disorders that requires intensive care. Delay in the start of treatmets raises the mortality risk; therefore, every nurse should recognise the basic organ dysfunctions, alerting to additional help at the right time and taking life-saving measures. Because of the development of treatment methods and medical devices, we can now treat patients effectively without invasive respirators. Studies have also shown that noninvasive (NIV) treatments have less side effects than invasive ventilation treatments and are often much cheaper. The right treatment method reduces the risk of prolongation of treatment.

In this thesis, we focussed on the implementation of NIV treatment. Because of the differences between NIV devices, the goal and purpose of this thesis was to create a guidebook on the evaluation of NIV devices. Therefore, every nurse can use this guidebook as a help for starting NIV treatment, adjusting, and understanding the values. The objective of this thesis was to increase patient safety and nurses’ knowledge of NIV usage in acute respiratory failure. The task of this thesis was to determine how to treat acute respiratory failure with a non-invasive ventilator.

The guidebook consists different types of sheets and pictures explaining the differences between each adjustment and their meanings. This makes the guide-book easily and quickly accessible, even during treatment procedures. The guidebook can also be used to support teaching or training. This thesis was ordered by the Tampere University of Applied Sciences Degree Programme in Nursing; therefore, the rights of usages of this thesis belong to the Tampere University of Applied Sciences. About further suggestion on this subject might be to evaluate the practicality of the guidebook with quantitative or qualitative study.
