Potilaiden kokemuksia LVAD-tukihoidosta

Abstract

Tämän opinnäytetyön tarkoituksena oli tutkia potilaiden kokemuksia LVAD-apupumpusta sekä sitä millainen merkitys vertaistuella ja sairaanhoitajan antamalla ohjauksella on potilaan hoidon, henkisen jaksamisen ja pärjäämisen kannalta. Tavoitteena oli tuoda esille uutta tietoa apupumpusta potilaiden näkökulmasta toimeksiantajalle, jota toimeksiantaja voi jatkossa käyttää ja näin ollen lisätä tietoisuutta apupumpusta ja sen vaikutuksista. Tavoitteena oli myös syventää omaa sairaanhoitajan osaamista sydänsairauksien ja niiden moniulotteisien vaikutuksien kannalta.

Keskeisiä tietoperustan käsitteitä opinnäytetyössä olivat sydämen vajaatoiminta, sen oireet ja seuraukset sekä eri hoitomuodot. Tietoa kerättiin LVAD-apupumpusta, sen asennuksesta sekä apupumppu hoidosta. Tietoa kerättiin myös apupumppupotilaan elämän muutoksista, ohjauksesta sekä vertaistuesta ja sen merkityksestä. Tietoperustan tueksi käytimme useita luotettavia lähteitä.

Tutkimus toteutettiin laadullisena tutkimuksena ja syvähaastattelimme tutkimusta varten 7:ää potilasta. Päädyimme laadulliseen tutkimukseen pienen potilasmäärän vuoksi sillä määrällinen tutkimusmenetelmä olisi ollut hyvin rajattua. Päädyimme laadulliseen tutkimukseen myös sen vuoksi, että se auttaa tuomaan apupumppupotilaiden henkilökohtaiset kokemukset ja tuntemukset paremmin esille. Haastatteluissa potilaat saivat kertoa tarinansa ja kokemuksensa omin sanoin. Tarkentavia kysymyksiä esitettiin tarvittaessa, mutta pääosin keskustelu oli luontevaa vuoropuhelua. Tutkimuksen aineisto analysoitiin aineistolähtöisen sisällönanalyysin menetelmällä. Tutkimuksen tuloksista tuli esille LVAD-apupumpun moninaiset vaikutukset sekä muutokset potilaan elämään, muutoksia tuli erityisesti arkisiin askareisiin. Tuloksista tuli esille myös se, kuinka suuri vaikutus laadukkaalla ohjauksella on siihen, kuinka hyvin potilas sopeutuu ja sitoutuu uuteen elämään apupumpun kanssa. Tuloksissa kuvataan, millaisia muutoksia potilaat kokivat ja ratkaisuja joita he ehdottivat epäkohtiin, joita he olivat kokeneet.

The purpose of this thesis was to explore patients’ experiences of the LVAD pump and the role of peer support and nurse guidance when it comes to the patients’ care, mental wellbeing and coping. The aim was to provide the commissioner with new information about the LVAD pump from the patient’s perspective, which the commissioner can use in the future and thus promote awareness of the LVAD pump and its effects. The aim was also to deepen our own knowledge of heart diseases and their multidimensional effects.

The key knowledge base concepts in the thesis were heart failure, its symptoms and consequences and the different treatment options. Information was collected on the LVAD pump, its installation and assist pump treatment. Information was also collected on the changes in the life of a patient with an LVAD pump as well as on the importance of guidance and peer support. Several reliable sources were used to support the knowledge base.

The study was conducted as a qualitative study and we conducted in-depth interviews with 7 patients. We opted for a qualitative study due to the small number of patients as a quantitative study method would have been very limited. We also chose qualitative research as it brings out the personal experiences and feelings of LVAD pump patients more clearly. In the interviews patients were encouraged to tell their stories and experiences in their own words. Clarifying questions were asked when necessary, but for the most part the discussion was a natural dialogue. The data was analysed using a data-driven content analysis method. The results of the study revealed the multidimensional effects of LVAD pump and the changes in the patient’s life, especially when it comes to normal everyday activities. The results also showed the impact of quality guidance and how well the patient adapts with their new life with the assistive pump. The results describe the changes the patients experienced and the solutions they suggested to the problems they had faced.
