Asukkaiden osallisuus hoitotyön arjessa

Abstract

Tutkimuksen tarkoituksena oli selvittää asukkaiden kokemuksia siitä, miten osallisuus ilmenee hoitotyön arjessa. Tavoitteena oli saada tietoa, jota voidaan hyödyntää kehitettäessä hoidon laatua palvelukodissa.

Tutkimuksessa kuvataan asukkaiden kokemuksia osallisuudesta. Teemoina ovat asukas palvelun kohteena, asukas palautteen antajana ja asukas osallisena palvelunsa ja hoitonsa kehittämisessä. Teemat perustuivat jo aikaisemmin muodostettuihin osallisuuden rooleihin ja niiden muotoihin. Tutkimus toteutettiin pääkaupunkiseudulla sijaitsevassa palvelukodissa.Tutkimuksen lähestymistapa oli laadullinen. Aineisto kerättiin teemahaastattelulla,haastattelemalla 9 asukasta. Aineisto analysoitiin deduktiivisella sisällön analyysillä.

Tutkimustulosten mukaan asukkaiden osallisuus arjessa liittyi päivittäisiin toimintoihin liittyviin arkirutiineihin, joissa oli yhtenä osana päihteettömyys. Suurin osa koki tekevänsä itsenäisiä päätöksiä arjessa. Esiin tuli kuitenkin sosiaalista arkuutta ja omien toiveiden vähättelyä. Yhteydet omaisiin olivat vähäisiä, mistä johtuen koettiin eristäytyneisyyttä. Asukkaiden asiantuntijuus ja tiedon hallinta liittyi asukkaiden tietämykseen sairauksiensa lääkehoidosta.

Pääasiassa asukkaat olivat tyytyväisiä palvelukodissa asumiseen. Asuminen koettiin asianmukaisena,turvallisena ja hyvänä paikkana olla ja elää. Hoitosuhteessa korostui vuorovaikutuksen tärkeys. Hyvän vastuuhoitajan ominaisuuksina korostettiin aitoa, välittävää ja keskustelevaa vuorovaikutusta, johon liittyivät hoitajan henkilökohtaiset ominaisuudet ja arvoperusta. Osallisuus hoitosuunnitelmien laatimisessa oli vähäistä tai sitä ei tiedostettu.

Asukaskeskeisyydessä osallisuus oli vuorovaikutussuhteiden ylläpitämistä muiden asukkaiden kanssa. Asukaskeskeisyys vastuuhoitajuuden näkökulmasta katsottuna jäi tässä tutkimuksessa epäselväksi. Osa tiesi vastuuhoitajansa, osa ei. Asukkaiden mieltymykset ja toimenpide-ehdotukset kohdistuivat pääasiassa aktiivisen arjen tukemiseen. Esiin tuotiin myös, että mahdollisuutta osallisuuteen olisi, jos mielenkiitoa riittäisi.

Aineiston perusteella jatkokehittämiskohteiksi muodostuivat: 1. Asukkaiden mukaan ottaminen oman hoitonsa suunnitteluun ja palveluiden kehittämistyöhön. 2. Aktiivisen arjen monipuolistaminen ja lisääminen asukkaiden toiveiden ja mieltymysten mukaisesti. 3. Hoitotyöhön liittyvien arvojen ja periaatteiden sisäistäminen vastuuhoitajuudessa.

The purpose of this study was to find out information concerning the experiences of the residents involvement in daily care. The aim of this study was to get information to be used in developing the care quality in service home.

The study depicts residents experiences of their involvement. The themes are a resident as a subject of the service, a resident as a contributor and a resident as a participant in developing his/hers service and care. The themes were based on the roles and forms of involvement made earlier. The study was carried out in a service home at metropolitan area. The approach of this study was qualitative. The data was collected with thematic interviews, a total of nine residents were interviewed. The data was analysed by using deductive analysis of the contents.

Based on the results of the study, the involvement of the residents in everyday life was connected with routines related to everyday functions, a part of them was abstinence. Most of the residents experienced to make decisions of their own in everyday life. However,it appeared social tenderness and deprecation of their own expectations. Connections with relatives were low, causing feeling of isolation. The expertise of residents and their control of information were related to the knowledge of their diseases medication.

Mostly the residents were satisfied in living at service home. They experienced that their residence was proper, safe and decent place to be and live in. They underlined the importance of interaction in the relation of care. They also underlined that the features of the decent responsible nurse were genuine, caring and conversational interaction, connected with personal attitude and set of values. Involvement at creation of care plan was slight or unconscious. Involvement in resident centricity was to maintain the interaction relationships between other inhabitants. In this study the resident centricity from responsible nurses perspective remained unclear. Some of the residents knew their responsible nurse,some did not. Residents preferences and suggestions of actions were mainly focused on supporting the activity in everyday life.

On the basis of this material the developing targets were: 1. To design residents care and developing services together with residents themselves. 2. To diversify and increase the active everyday life due to residents desires and references. 3. To internalize the values and principles in the treatment as a part of responsible nursing.
