Potilaan toiveet liittyen sädehoito-osaston fyysiseen hoitoympäristöön

Abstract

Opinnäytetyön tarkoituksena oli selvittää syöpäpotilaan toiveita HYKS Syöpätautien klinikan sädehoito-osaston fyysiseen hoitoympäristöön liittyen. Tavoitteena oli tuottaa tietoa tulevan HYKS Syöpätautien klinikan sädehoito-osaston fyysisen hoitoympäristön kehittämiseksi potilaslähtöisestä näkökulmasta. Opinnäytetyö oli osa HYKS Trauma- ja Syöpäkeskushanketta, jonka tarkoituksena on rakentaa uusi sairaala, johon myös suurin osa Syöpätautien klinikan toiminnasta siirtyy. Sädehoito-osaston tiloja haluttiin uudistaa potilaslähtöisestä näkökulmasta. Opinnäytetyössä tutkittiin potilaan toiveita liittyen fyysisen hoitoympäristön eri osa-alueisiin. Fyysinen hoitoympäristö voidaan jakaa toiminnallisiin tekijöihin, fyysisiin tekijöihin ja tunnetekijöihin. Toiminnallisiin tekijöihin kuuluvat tilojen ahtaus tai väljyys ja potilaan intimiteetin ja yksityisyyden mahdollistaminen. Fyysisiin tekijöihin luetaan muun muassa hoitoympäristön ilman laatu ja ääniympäristö kuten meteli tai hiljaisuus. Tunnetekijöitä hoitoympäristössä taas ovat tilojen viihtyisyys, kodikkuus, turvallisuus ja intimiteetin toteutuminen. Opinnäytetyössä tutkittiin myös potilaan turvallisuudentunteen toteutumista hoidon aikana. Opinnäytetyötä varten luotiin kyselylomake, joka esitestatiin HYKS Syöpätautien klinikan sädehoito-osastolla tammikuussa 2015 (N=20). Opinnäytetyön aineisto kerättiin sädehoitopotilailta (N=120) kyselylomakkeella aikavälillä tammikuu – hemikuu 2015. Kyselylomakkeeseen vastasi 88 potilasta. Kyselylomakkeessa oli sekä suljettuja että avoimia kysymyksiä. Suljetuista kysymyksistä saatu kvantitatiivinen aineisto analysoitiin IBM SPSS Statistics 21 –ohjelmalla. Avointen kysymysten vastaukset analysoitiin teoriaohjaavalla sisällönanalyysillä. Kyselyyn osallistuneiden potilaiden vastauksista kävi ilmi, että potilaat toivoivat sädehoito-osaston odotustiloihin lisää viihtyvyyttä lisääviä tekijöitä, kuten viherkasveja, värejä ja aika-kausilehtiä. Odotustilojen istuinten toivottiin olevan tukevampia ja korkeampia. Hoidon aikana potilaat pitivät omavalintaisen musiikin kuuntelua tärkeänä. Potilaiden kokemaan turvallisuudentunteeseen sädehoitohuoneessa vaikutti eniten hoitohenkilökunta.

The purpose of the final project was to explore cancer patient´s wishes concerning physical nursing environment of HUCH Department of Oncology Radiation Therapy Ward. The aim of the final project was to bring new information on patient-based perspective and help develop the physical nursing environment of new HUCH Department of Oncology Radiation Therapy Ward. The final project was part of HUCH Trauma and Cancer Center project. The purpose of HUCH project is to build a new hospital to where most the activity of the Department of Oncology will be relocated. The facilities of the Radiation Therapy Ward were desired to be renewed on a patient-based perspective. In the final project patients’ wishes were studied regarding different sectors of physical nurs-ing environment. Physical environment can be divided into functional factors, physical factors and emotional factors. Functional factors include the narrowness or spaciousness of the facilities and factors that enable patients’ intimacy or privacy. Physical factors include for example the quality of air and the noisiness or quietness of the environment. Emotional factors in nursing environment are the coziness and homeliness of the environment and implementation of security and intimacy. The questionnaire form made for the final project was pretested at the HUCH Department of Oncology Radiation Therapy Ward in January 2015 (N=20). The data of the final project were collected from cancer patients (N=120) in January – February 2015 with a question-naire form. The questionnaire was returned by 88 patients. The questionnaire form included both closed-ended and open-ended questions. The quantitative data of the closedended questions were analyzed by using the IBM SPSS Statistics 21 program. The qualitative data of the open-ended questions were analyzed by combining inductive and deductive content analysis. The results of the final project showed that patients wished for more factors that would increase the coziness of the waiting area, like indoor plants, colors and magazines. Patients wished for steadier and higher seats. During the radiotherapy patients found important that they could choose the music they listened to. Patients’ feeling of security in the radiotherapy room was mostly influenced by the nursing staff. Keywords Physical nursing environment, Radiation Therapy Ward
