ORTOREKSIA : keskustelumalli opiskeluterveydenhuoltoon

Abstract

Ortoreksia nervosa on Steven Bratmanin esille tuoma epätyypillinen syömishäiriö, jossa terveellinen ruoka muuttuu pakkomielteeksi. Ortoreksianomaisella syömis-käyttäytymisellä pyritään puhtaiden ja terveellisten ruoka-aineiden avulla terveeseen elämään. Ortorektikon ruokavalio kaventuu, kun ruokavalioon sallitaan vain tietyt ruoka-aineet. Tämä altistaa erilaisille terveyshaitoille, kuten yleiskunnon heikkenemiseen ja aineenvaihdunnan hidastumiseen.

Toiminnallisen opinnäytetyön tavoitteena ja tehtävänä oli luoda työväline opiskelu-terveydenhuollossa työskenteleville terveydenhoitajille ortoreksian tunnistamisen ja puheeksi ottamisen avuksi asiakastilanteessa. Keskustelumalli on kahden sivun pituinen motivoivan haastattelun hengessä tehty keskustelumalli, joka sisältää pää- ja lisäkysymysten lisäksi tieto-osiota ortoreksiasta. Terveydenhoitajilta tulleen palautteen mukaan sekä toimeksiantajan mielestä luotu keskustelumalli on hyödyllinen keskustelun tuen ja tiedon antajana asiakastilanteissa.

Keskustelumallin tietoperusta on peräisin opinnäytetyön kirjallisesta osiosta. Keskustelumallin kysymykset on rakennettu ORTO-15- kyselyä ja Steven Bratmanin kysymyksiä apuna käyttäen. Jatkokehitysidea opinnäytetyöhön voisi olla opas terveydenhoitajille ortoreksian ja muiden syömishäiriöiden ennaltaehkäisystä tai nuorille suunnattu luento syömishäiriöistä ja niiden ilmenemisestä. Luento sisältäisi nuorten kesken ryhmäkeskustelua syömishäiriöistä ja omaan kehoon suhtautumisesta.

Orthorexia nervosa, introduced by Steven Bratman, is an atypical eating disorder, in which healthy food becomes an obsession. Orthorexic eating behaviour is characterised by strive for a healthy life by merely eating healthy and clean food. The diet of an orthorexic becomes restricted as only certain foods are allowed. This predisposes to various health hazards, such as deterioration of general condition and slowing down of metabolism.

The aim of this practice-based thesis was to create a tool for public health nurses in Student Health Care Services to help recognize orthorexia and bring up the subject in client work. A conversational model based on motivational interviewing was created. In addition to main and additional questions, it presents facts about orthorexia. According to the feedback from public health nurses, the conversational model is useful for supporting the conversation and giving information in client work.

The knowledge base of the conversational model is presented in the theoretical part of the thesis. The questions were compiled from the ORTO-15 questionnaire and Steven Bratman’s questions. A guide for public health nurses about the prevention of orthorexia and other eating disorders could be a further development idea. Also, a lecture for the youth about eating disorders and their occurrence, including group conversations about eating disorders and attitudes towards one’s own body, might be useful.
