Tiivistelmä
Häpeä on tunne, jolla on osuutensa monessa terveyteen ja mielenterveyteen liittyvässä haasteessa. Päihdeongelmaan, etenkin sen liittyessä vanhemmuuteen, kuuluu tyypillisesti vaikea häpeän tunne. Se aiheuttaa eristäytymistä ja syventää riippuvuuden syöksykierrettä. Häpeän hoitaminen onkin tärkeä osa päihdeongelmasta toipumista. Pahimmillaan häpeä voi kuitenkin estää avun hakemisen ja vastaanottamisen. Jotta häpeää ja sitä kautta päihdeongelmaa voidaan hoitaa, tarvitaan erilaisia keinoja lähestyä ja käsitellä aihetta. Tämä opinnäytetyö esittelee tähän yhden vaihtoehdon: ryhmämuotoisen draamaprosessin.
Soveltava draama valikoitui menetelmäksi siksi, että taidemenetelmät ovat tutkitusti toimiva keino käsitellä asioita joista puhuminen on vaikeaa. Tarkempi menetelmä, prosessidraama, on poikkitaiteellinen kokonaisuus, jossa ei valmisteta esitystä muille, vaan tutkitaan ryhmänä jotakin ilmiötä ja teemaa. Tämä tehdään esimerkiksi erilaisin kuvataiteellisin, teatterillisin, tanssillisin tai äänenkäytöllisin keinoin, jonka jälkeen havaintoja reflektoidaan yhdessä pohtien. Tässä kyseisessä prosessissa tutkittiin häpeää sekä sen vastavoimaa myötätuntoa eri näkökulmista vertaistukea hyödyntäen ja korostaen.
Opinnäytetyön tilaaja on Helsingin ensikoti ry. Sen yhteydessä toimivat Pidä kiinni-yksiköt toteuttavat vauvalähtöistä päihdekuntoutusta niin ympärivuorokautisissa yksiköissä kuin avopalveluissakin. Häpeä tunnistetaan niissä merkittäväksi teemaksi niin päihdetoipujien kuin päihdetyötä tekevien osalta. Suunnitelma prosessidraamasta rakennettiin päihde- ja mielenterveystyön sekä soveltavan taiteen ammattilaisten haastatteluiden sekä teoriatiedon pohjalta. Tämän jälkeen ryhmä testattiin käytännössä Helsingin ensikodin Pidä kiinni-yksiköiden asiakkaille. Sen aikana tehtiin havaintoja ja kerättiin palautetta toiminnasta.
Tämän tutkimustyön tulosten perusteella voi sanoa, että draaman menetelmät sopivat hyvin häpeän hoitoon motivaation ollessa korkea ja puitteiden mahdollistavan osallistumisen. Luovat sekä toiminnalliset menetelmät sen käsittelyyn koetaan hyviksi. Kehittämistyöni nostaa esiin häpeästä puhumisen tärkeyttä ja tarjoaa konkreettisia ehdotuksia sen käsittelyyn päihdekuntoutuksen tueksi. Ne ovat käyttökelpoisia myös muissa ryhmäprosesseissa, joissa häpeä on oleellinen teema käsitellä. When it comes to substance use disorder, shame plays a notable role. Adding to it ,parenthood makes it an even bigger challenge. Shame can cause isolation and deepen the circle of addiction. In order for recovery to be possible, it is important to understand and treat them simultaneously. In the worst case, shame may prevent individuals from seeking and receiving help. Therefore, in order to address shame and thereby treat substance use disorder, different approaches are needed. This thesis presents one possible approach: group format process drama.
Art methods are often a beneficial way to deal with things that are difficult to put into words. Applied drama was selected as a working method based on my personal experience in the field. More specifically, process drama is an interdisciplinary approach, where the goal is not to prepare a play or a show for someone else to watch, but rather to study some specific theme as a group. It incorporates methods from dance to visual arts and theatre.
The Helsinki Mother and Child Home Association offers baby-oriented substance abuse disorder rehabilitation. Both clients and staff members in these units recognize the challenges associated with shame and consider them to be a significant factor. For the purposes of the current thesis, I designed a process drama for a small group to study shame and its counterforce, compassion. We surveyed these topics from different angles while utilizing and emphasizing peer support.
Drawing on the literature and interview data, a plan was developed for the group. Interviews were conducted with substance abuse- mental health- and applied theatre specialists. The process drama was tested with clients of the Helsinki Mother and Child Home Association. Observations and feedback were collected concurrently and afterward. Findings showed that drama is a beneficial method to address shame, when motivation is high and the setting promotes participation. The current work highlights the importance of talking about shame and offers concrete suggestions of how to approach it, with regards to substance use disorder. Finally, on the one hand this approach is promising and should explored more in these particular units and on the other hand, it is worth exploring the methods in other group processes where shame plays a role.
