Tiivistelmä
Opinnäytetyön tarkoitus oli kartoittaa kommunikointia 0-6-vuotiaan lapsipotilaan ja sairaanhoita-jan välillä ja siihen liittyviä hyötyjä ja haasteita. Tavoitteena opinnäytetyössä on edistää kommu-nikointia lapsipotilaiden ja sairaanhoitajan välillä. Opinnäytetyön toimeksiantajana toimi Turun ammattikorkeakoulu ja opinnäytetyö toteutettiin narratiivisena kirjallisuuskatsauksena (n=25) ja analysoitiin sisällön analyysin avulla.
Lapsen hyvinvoinnin eräitä peruspilareita ovat kyky kommunikoida, käyttää kieltä ja puhua. Kommunikoinnin hallinnan valmiudet kehittyvät jo raskauden aikana. Myönteisessä kommuni-kaatiossa imeväisiässä ilmaantuvat lapselle perusvalmiudet myöhemmille vuorovaikutustaidoille. Lapsen oikeuksien sopimuksen noudattaminen on kaikkien velvollisuus ja oikeudet kuuluvat jokaisella lapselle.
Imeväisikäisellä kommunikointi perustuu reflekseihin ja painottuu ruumiilliseen viestintään. Varhaisleikki-ikäisen kommunikointi on sekoitus sanatonta ja sanallista viestintää. Myöhäisleikki-ikäinen puhuu selkeästi ja kykenee keskusteluun toisten kanssa. Lapsen keskeisiä kommuni-koinnin välineitä ovat leikki, musiikki ja kuvataide. Tehokas hoitotyö perustuu kommunikointiin ja sairaanhoitajan taitoihin olla vuorovaikutuksessa potilaan kanssa. Kommunikaatio lapsipotilaan ja hänen vanhempiensa kanssa voi olla yksi haastavimmista tehtävistä osana lasten hoitotyötä. Haasteita lasten hoitotyössä ovat lapsen osallistaminen omaan hoitoonsa, kommunikointi lapselle ikä- ja kehitystasoa vastaavasti ja lapsen oikeuksien toteutuminen hoidossa. Lapsen kanssa kommunikoinnissa on otettava huomioon lapsen ikä, kehitystaso, aikaisemmat sairaalakokemukset, sanojen ymmärrettävyys ja lapsen oikeus osallistua omaan hoitoonsa. Positiivinen kommunikointi vähentää lapsen kokemaa pelkoa, kivun tuntemusta, stressiä, ahdistusta, ennaltaehkäisee lapsen masennusta ja edistää lapsen hyvinvointia ja parantumista.
Jatkotutkimus aiheena voisi kartoittaa kommunikointia erityistarpeisen lapsen kanssa, kuten kehitysvammaisen, kuuron tai sokean sekä kommunikointia yli 6-vuotiaiden lasten kanssa. The purpose of this thesis was to review communication with a child patient aged 0-6 years and what kind of benefits and challenges appears from the communication. The objective is to improve on the communication between pediatric patients and nurses. The thesis was commissioned by Turku University of Applied Sciences and is implemented as a narrative literature review (n=25) and analyzed by a content analysis.
Bases of the child´s well-being are the ability to communicate, use language and speak. Communication management skills are already developing during the pregnancy. In positive communication, the infant develops basic skills for later interaction skills. Respecting the Convention on the Rights of the Child is everyone's responsibility and the rights belong to every child.
In infancy, communication is reflexive and focuses on the physical interaction. In the early childhood childrens communication is a mixture of verbal and nonverbal communication. In the later childhood the child speaks clearly and is able to discuss with others. The child's main communication tools are to play, play music and to do visual arts. Effective nursing is based on communication and the ability of the nurse to interact with the patient. Communicating with a pediatric patient and their parents can be one of the most challenging tasks as part of pediatric nursing. Challenges in pediatric nursing include involving the child in it’s own care, communicating to the child according to it’s age and developmental level, and realizing the child's rights in the care. Communicating with a child patient, should you take into consideration the child's age, level of development, previous hospital experience, comprehensibility of words and the child's right to participate in their own care. Positive communication decreases the child`s experience in fear, pain, stress and anxiety. It also prevents depression and promotes the child`s well-being and healing.
As a further research on the subject could examine the communication with a child with special needs such as blindness, deafness or being mentally handicapped and examine the communication with a child patient over six years old.
