Opinnäytetyössä tutkittiin toipumisorientaation toteutumista Helsingissä sijaitsevan psykiatrisen sairaalan kuntouttavalla osastolla. Tutkimuksessa selvitettiin, mitä toipumisen on koettu tarkoittavan sekä toipumisorientaation toteutumista tukevia ja estäviä tekijöitä. Toipumisorientaation toteutumista on aiemmin tutkittu Suomessa vain vähän.
Laadullisen opinnäytetyön aineisto koostui henkilökunnan ryhmähaastatteluista ja potilaiden yksilöhaastatteluista. Haastattelut toteutettiin puolistrukturoituina teemahaastatteluina ja haastatteluista saatu aineisto analysoitiin yhdistäen teoriaohjaavaa sekä aineistolähtöistä lähestymistapaa. Toipumisorientaation toteutuminen liittyi neljään luokkaan: hoito ja osaston rakenteet, vuorovaikutus ja ilmapiiri, potilas ja sairaus sekä kotiympäristö.
Haastatteluissa ilmeni runsaasti toipumisorientaatioon liittyviä tekijöitä, joiden perusteella toipumisorientaatio näytti toteutuneen osastolla hyvin. Henkilökunta kuvasi toteuttaneensa toipumista tukevaa voimavarakeskeistä hoitoa osastolla jo vuosia. Henkilökunnan keskuudessa esiintyi kuitenkin merkittävää epävarmuutta siitä, mitä toipumisorientaation viitekehyksellä tarkoitetaan ja miten sitä toteutetaan käytännössä. Toipumisen kokemisessa henkilökunnan ja potilaiden välillä ei ilmennyt merkittäviä eroavaisuuksia. Henkilökunta kuvasi toipumista yleisempien käsitteiden kautta, kun taas potilaat yksityiskohtaisemmin omien kokemustensa kautta.
Tutkimuksessa keskeisimmät toipumisorientaation toteutumista tukevat tekijät olivat henkilökunnan ja potilaiden mukaan kuulluksi tuleminen, mielekäs tekeminen, ryhmät, läheisten tuki sekä toivon ylläpitäminen. Keskeisimmät estävät tekijät olivat sairauden oireet, pelot ja läheisten tuen puuttuminen. Merkittävänä toipumisorientaation toteutumista tukevana tekijänä näyttäytyi osaston hyvä ilmapiiri. Tutkimuksen tuloksia voidaan käyttää jatkossa toipumisorientaatioon liittyvän tutkimus- ja kehittämistyön apuna. Jatkotutkimusaiheina tämän tutkimuksen perusteella voisi olla suomalaisen kulttuurin vaikutuksen tutkiminen jatkossa liittyen potilaiden kokemaan kuormitukseen ryhmämuotoisissa hoidoissa sekä nyt löydetyn nelikentän yleistettävyyden testaaminen toipumisorientaation toteutumisen kannalta.

The purpose of this study was to find out how recovery orientation had actualised in a rehabilitating ward in one of the psychiatric hospitals in Helsinki. The other aim of the thesis was to find out what factors had been supporting or blocking recovery orientation in that ward and how recovery was experienced by the patients and the staff members. Prior to this study, only a few studies had been conducted on recovery orientation in Finland.
The study was qualitative. The material was collected by interviewing the staff members and patients of the ward by using theme interviews as a method. Group interviews were conducted with the staff members, and the patients were interviewed individually. The material was analysed by combining theory- and databased analysing styles. According to this study, the actualisation of recovery orientation was associated with four categories: care and structures of the ward, interaction and atmosphere, patient and illness and home environment.
The data showed that there were numerous factors supporting recovery orientation in that ward, which indicated that recovery orientation had actualised there well. The staff members reported having practiced same kind of resource-centred care for years. Among the staff members there was uncertainty about what recovery orientation actually meant and how it should be carried out in practice. There were no significant differences between the staff members’ and patients’ experiences about the meaning of recovery. The personnel used more general terms when describing recovery, whereas the patients relied more on their personal, more detailed experiences.
In this study, the most supporting factors of recovery orientation according to both staff members and patients were the following: patient’s feeling of being heard, doing meaningful things, group activities, support of loved ones, and maintaining hope. The most blocking factors were symptoms of the illness, fears and lack of support from loved ones. A significant supporting factor was a good atmosphere in the ward. The conclusions of this study can be used in developing functioning and implementing recovery-oriented care. For the future research it would be interesting to know more about the impact of Finnish culture relating to a feeling of burden by patients in group activities. It could also be interesting to test whether the four-field category found in this study could be used more widely when assessing or studying recovery orientation.
