Tiivistelmä
Tämän opinnäytetyön aihe oli ikääntyneen haavanhoito-opas. Opinnäytetyön tavoite oli tuottaa ikääntyneelle kotiin annettava haavanhoito-opas. Toiminnallinen opinnäytetyö toteutettiin yhteistyössä länsisuomalaisen terveyskeskuksen kanssa. Tavoite on antaa potilaalle tukea haavan hoitoon kotona. Terveyskeskuksesta saadut suulliset ohjeet voivat helposti unohtua, joten opas on muistuttamassa potilasta, miten haavaa pitää hoitaa.
Opinnäytetyön raportissa käydään läpi yleisimpiä ikääntyneillä esiintyviä haavoja ja niiden aseptista hoitoa, haavan väriluokitusta, haavanhoitotuotteita, haavan paranemisprosessia sekä kirjaamista. Oppaassa esitellyt haavat ovat sellaisia, joita yhteistyökumppani toivoi, sillä ne ovat yleisimmin kyseisellä terveyskeskuksella esiintyviä haavoja. Tässä opinnäytetyön raportissa tarkastellaan samoja haavoja. Opinnäytetyön raportissa käymme myös läpi sairaanhoitajan ja sairaanhoitajadiakonissan antamaa tukea potilaalle.
Opinnäytetyön tekoprosessi oli pitkä. Aloitimme opinnäytetyön suunnittelun reilu vuosi sitten. Opinnäytetyöprojektin aikana olemme olleet yhteydessä yhteistyökumppaniin ja saaneet kehitys- ja muokkausideoita. Haavanhoito-opas on toimitettu yhteistyökumppanille PDF-muodossa. The subject of this thesis was Wound care guide for the elderly. The thesis has been implemented in collaboration with the healthcare centre located in western Finland. The aim was to provide patients with support and guidance in self-care of wounds at home after being discharged from the healthcare centre. These instructions serve as a reminder to the client about appropriate at-home wound treatment because the oral instruction from the healthcare centre can be easily forgotten.
The report of this thesis covers the most prevalent wounds among the elder population. Furthermore, it describes aseptic care, the color scale of wounds, wound care products, the process of recovery and documentation. The collaboration partner proposed the se- lected wounds, which have been discussed in the wound care guide, the selected wounds being the most commonplace. The report of this thesis describes the same wounds in further detail. Within the report, we additionally cover the support given to the client by a nurse and a diaconal nurse.
The process of this thesis work lasted long period of time. The whole process took over a year to complete. During the process of the thesis we have been in contact with the collaboration partner and received development and editing ideas. The Wound care guide has been submitted to the partner in PDF format.
