Tiivistelmä

Opinnäytetyön tarkoituksena oli päivittää Joroisten vuodeosaston kotiutusohjeisto työnantajan toivomuksesta. Tutkimustehtävämme oli selvittää miten hoitajat kokivat kaavakkeen muuttamisen tarpeellisuuden kotiutuksen suunnittelussa, mitkä ovat kotiutumisen kannalta tärkeimpiä huomioitavia asioita ja keitä hoitajat toivoisivat olevan paikalla kotiutuspalaverissa, huomioiden moniammatillisuuden. Päivittämistarve ilmeni keskustelussa vuodeosaston esimiehen kanssa. Työn teoriaosuudessa selvitettiin kotiutukseen liittyviä hoitotyön eettisiä periaatteita ja moniammatillista yhteistyötä.
Tutkimuksen haastattelut toteutettiin kahtena erillisenä parihaastatteluna. Ensimmäisenä haastateltiin palveluohjaajaa ja kotihoidon sairaanhoitajaa. Jälkimmäisenä haastateltiin kahta vuodeosaston sairaanhoitajaa. Haastattelut tehtiin helmikuussa 2012. Tutkimuksemme toteutettiin kvalitatiivista tutkimusmenetelmää käyttäen. Teemahaastattelut analysoimme sisällönanalyysiä käyttäen.
Tulokseksi saatiin haastattelujen perusteella, että kaavakkeen päivittäminen oli ajankohtaista. Tärkeiksi huomioitaviksi asioiksi haastatteluista nousivat, että kotiutuksen suunnittelu alkaa jo potilaan tullessa osastolle kunnioittaen hänen tahtoaan. Hoidon jatkuvuus turvataan moniammatillisella yhteistyöllä.
Tutkimuksemme toivotaan tulevaisuudessa hyödyttävän niin vuodeosastoa, kotihoitoa kuin kotiutuvaa potilastakin.

The aim of this study is to update the instructions of discharge of the ward of the Joroinen hospital. The research task was to find out how nurses experienced the necessity of a withdrawal form to change the design, what are the discharges from the most important points to consider and nurses who would like to be a meeting place of discharge, taking into account the multi-professionalism. Need for the update was revealed while interviewing the principal of the ward.
The questioning was carried out in two separate stages in means of face to face interviewing in pars. First ones to be interviewed were the service director and the nurse of the domiciliary care. The latter interview was with two nurses of the hospital ward. The questioning commenced in February 2012. Analysis of this study was carried out with a qualitative method. Topic based interviews were analyzed by content analysis.
The results of these interviews show, that the form was updated topical. As important to make note of issues of the interviews went up, that the planning of the patient discharge from the hospital begins when the patients comes to the ward, with the respect of his will. The continuity of the treatment is ensured with multi-professional collaboration.
We hope that this study would benefit both the ward as well as domiciliary care. In the theory part of this study, we determined the ethical principals of discharge from the hospital and multi-professional collaboration.
