Tieto lastenneuvolan terveydenhoitajan työssä

Abstract

Tämä opinnäytetyö toteutettiin osana Laurea-ammattikorkeakoulun ja Aalto-yliopiston yhteistä MORFEUS-hanketta. Hankkeen tarkoituksena on kehittää hyvinvointipalvelusektorin toimijoiden välistä yhteistyötä ja tiedonjakoa. Tarkastelun ja kehittämisen kohteena hankkeessa ovat päihde-, mielenterveys- ja lastensuojelupalveluiden kokonaisuudet.

Opinnäytetyö rajattiin koskemaan hankkeen lastensuojelupalveluiden kokonaisuutta. Näkökulmaksi valittiin lastenneuvolan terveydenhoitajien toiminta sekä ennaltaehkäisevässä lastensuojelutyössä että tilanteissa joissa lapsi tai perhe on jo lastensuojelun asiakas. Tavoitteena oli tutkia mitä asiakasta koskevia tietoja lastenneuvolan terveydenhoitajat tarvitsevat tukeakseen lasta ja tämän perhettä, mitä kautta tietoja saadaan, sekä mitä haasteita tietojen kulussa on.

Laadullisena tutkimuksena toteutettuun tutkimukseen valittiin viisi Porvoon äitiys- ja lastenneuvolan terveydenhoitajaa. Tiedonhankintamenetelmänä käytettiin stimuloitua teemahaastattelua. Haastattelut nauhoitettiin, purettiin kirjalliseen muotoon ja analysoitiin aineistolähtöisellä sisällönanalyysilla. Tutkimustuloksissa nousi esille lapsen vanhempien tärkeys koko perheen hyvinvoinnin edistäjinä. Terveydenhoitajat kuvasivat tarvitsevansa tietoa erityisesti vanhempien voimavaroista, sekä vanhemmuuden ja lapsen kasvatuksen toteutumisesta. Tietoja saadaan asiakkaan itsensä kertoman kautta sekä perheen tukemiseen osallistuvilta yhteistyötahoilta. Asiakkaan kertoman todenmukaisuutta joudutaan kuitenkin ajoittain arvioimaan. Lisäksi terveydenhoitajat kokivat, etteivät he saa muilta toimijoilta tarpeeksi tietoa asiakkaan tilanteesta tai perheelle annetuista tukitoimista. Tiedon kulkuun vaikuttivat oleellisesti puutteet yhteistyö- ja tiedonvaihtokäytänteissä, toimijoiden tekemät tulkinnat salassapitovelvollisuudesta, sekä kiire ja toimijoille yhteisen tietoverkon puuttuminen.

Tämän opinnäytetyön tuloksia voidaan hyödyntää muun muassa MORFEUS-hankkeen toteuttamisessa, mitä kautta tulokset palvelevat myös hyvinvointipalvelusektorin toimijoiden yhteistyön kehittämistä. Lisäksi työssä on esitetty kehitysehdotuksia lastenneuvolan ja muiden toimijoiden välisen yhteistyön ja tiedonkulun parantamiseksi.

This thesis was produced as a part of combined project called MORFEUS by Laurea University of Applied Sciences and Aalto University. The purpose of the project is to develop the co-operation and information sharing of actors in the wellbeing service sector. The project concentrates on exploring and developing services related to substance abuse, mental health and child protection.

This thesis focuses only on the child protection part of the project MORFEUS. The goal was to research the actions of public health nurses at child welfare clinics and find out what customer related information they need to support the child and his family, in which way they get it and what problems they face when trying to get the information. The study contains also preventive care and situations where the child or the family is a customer of child protection services.

The approach of this study was chosen to be qualitative. A stimulated thematic interview was used to gather information from five public health nurses who work in the child welfare clinic of Porvoo. Interviews were recorded, transcribed and analyzed by using inductive content analysis. The results of the study showed that parents of the child have a key role in supporting the welfare of the whole family. Public health nurses felt that they need information especially about the assets of the parents, parenthood and how the parents manage to raise the child. Information is gathered from the customer and co-operative actors. Interviewees however mentioned that they cannot always rely on the parents telling the truth regarding their family situations. According to public health nurses, other co-operators do not give enough information about the situation of the client and the supportive actions given to them. Results also indicate that lacking corporate- and information policies, personal opinions of confidentiality, haste at work and the lack of a shared data network all affected to the information flow between public health nurses and co-operators.

The results of this thesis can be utilized for example in the implementation of the project MORFEUS. In addition, the results also serve in developing the cooperation between different actors in the wellbeing service sector. Development ideas have been also introduced to improve the cooperation and information sharing between public health nurses and other actors.
