Vantaan kaupungin hoitajien osaamisen kartoitus akuuttien haavojen hoidossa

Abstract

Tämä opinnäytetyö toteutettiin yhteistyössä Vantaan kaupungin sosiaali- ja terveydenhuollon kanssa vuonna 2015- 2017 ja se on osa haavanhoitoprojektia. Vantaan kaupungin sosiaali- ja terveydenhuollossa haavanhoidon kehittämistä pidetään hyvin keskeisenä kehittämiskohteena kaupungin terveys- ja sairaalapalveluissa. Opinnäytetyön tarkoituksena oli selvittää, millaiseksi terveysasemilla työskentelevät sairaanhoitajat kokivat oman haavanhoito osaamisensa hoitaessaan akuutteja haavoja, mistä he hakivat lisätietoa haavanhoitoon ja miten he ohjasivat akuutin haavan saanutta potilasta. Opinnäytetyöllä saatiin ajankohtaista tietoa hoitajien todellisesta haavanhoito osaamisesta ja tavoitteena oli tuottaa hyödyllistä, konkreettista ja kehitystyötä eteenpäin vievää tietoa.

Opinnäytetyötä varten haastateltiin kuutta sairaanhoitajaa Vantaan kaupungin terveysasemilla. Tiedonkeruuvälineenä opinnäytetyössä käytettiin teemahaastattelua ja tutkimusaineiston analysoinnissa käytettiin aineistolähtöistä sisällönanalyysiä. Saatujen tulosten mukaan sairaanhoitajat kokivat osaavansa hoitaa akuutteja haavoja hyvin, eivätkä he kaivanneet lisäkoulutusta akuuttien haavojen hoitoon liittyen. Haastatellut sairaanhoitajat kokivat olevansa motivoituneita haavanhoitoon ja he toivoivat lisäkoulutusta koskien haavanhoitoa kokonaisuudessaan. Lisätietoa haavanhoitoon haettiin pääasiallisesti Terveysportista sen luotettavuuden ja nopeuden vuoksi. Kaupungin intrassa olevaa AVAINTA sairaanhoitajat käyttivät harvoin. Potilasohjausta haavan hoitoon liittyen toteutui terveysasemilla päivittäin ja suullisen ohjauksen tukena he käyttivät kirjallista materiaalia. Vastaavanlainen tutkimus soveltuisi myös muille terveysasemille tehtäväksi, näin saataisiin laajempaa tietoa hoitajien osaamisesta akuuttien haavojen hoidon osalta. Opinnäytetyöhön haastateltiin vain kuutta kaupungin sairaanhoitajaa, joten saatuja tuloksia ei voi yleistää kaikkia terveysasemia tai hoitajia koskevaksi.

This Bachelor’s thesis was carried out together with the Social and Health Care Services of the City of Vantaa in 2015-2017, and it is a part of a wound care project. The Social and Health Care Services of the City of Vantaa consider the development of wound care a target of great importance in developing the city’s health and hospital services. The purpose of a this thesis was to study how capable nurses employed at health stations felt they were of looking after acute wounds, where they sought further information on wound care, and how they guided a patient who has received an acute wound. The thesis provides current information on the nurses’ real wound care skills, the aim being to produce useful and concrete information that would also contribute to further development.

For the thesis, six nurses were interviewed at the health stations of the City of Vantaa. Information for the thesis was collected with a by a thematic interview, and the material was interpreted by data-driven content analysis. The findings indicate that the nurses felt they were well able to take care of acute wounds, with no need of further training. The nurses who were interviewed felt motivated to look after wounds, hoping to access additional instruction related to wound care as a whole. Further information on wound care was sought, primarily, at the Terveysportti portal because it is a reliable and easy way of accessing information. AVAIN (key) available on the Intranet of the city was seldom used by the nurses. Patients were given advice on wound care every day at the health stations, and, to support oral guidance, the nurses used written material. A corresponding study could be effected also at other health stations, with it yielding more extensive information on nurses’ acute wound care skills. For the thesis, only six city nurses were interviewed; hence, the findings cannot be generalized to apply to all health stations or nurses.
