Sytostaattihoitoa saavan keuhkosyöpäpotilaan ohjauksen prosessikuvaus

Abstract

Opinnäytetyö tehtiin Kolmiosairaala-hankkeessa. Kolmiosairaala on marraskuussa 2010 valmistunut sairaala Meilahden kampusalueella Meilahden tornisairaalan vieressä. Kolmiosairaalahankkeen yhteistyökumppaneina toimivat Helsingin ja Uudenmaan sairaanhoitopiirin HYKS:n Medisiininen tulosyksikkö ja Laurea-ammattikorkeakoulun Otaniemen paikallisyksikkö. Opinnäytetyön varsinaisina yhteistyökumppaneina toimivat Kolmiosairaalan Keuhkosairauksien klinikan vuodeosasto 6A, Päiväsairaala ja Keuhkosairauksien poliklinikka.

Opinnäytetyön tavoitteena on kehittää sytostaattihoitoa saavan keuhkosyöpäpotilaan ohjausta Meilahden Kolmiosairaalan keuhkosairauksien klinikalla, jossa henkilökunta voi myös hyödyntää opinnäytetyön prosessikuvausta paitsi potilasohjauksen kehittämisessä myös uusien työntekijöiden perehdyttämisessä. Opinnäytetyön tarkoituksena oli tuottaa sytostaattihoitoa saavan keuhkosyöpäpotilaan ohjauksesta prosessikuvaus Meilahden kolmiosairaalan keuhkosairauksien klinikalle.

Tämän opinnäytetyön tutkimusmenetelmänä oli kvalitatiivinen tutkimusmenetelmä. Aineisto kerättiin teemahaastattelulla, joka toteutettiin kolmen sairaanhoitajan ryhmähaastatteluna. Tutkimuskysymykset, joihin haettiin vastauksia, olivat seuraavat: mitkä ammattiryhmät osallistuvat sytostaattihoitoa saavan keuhkosyöpäpotilaan ohjaukseen ja millainen on sytostaattihoitoa saavan keuhkosyöpäpotilaan ohjausprosessi. Haastatteluteemoina olivat sytostaattihoitoa saavan keuhkosyöpäpotilaan ohjaukseen valmistautuminen, ohjauksen toteutus ja sisällöt sekä ohjauksen arviointi. Aineisto analysoitiin induktiivisella sisällönanalyysillä. Analyysin pohjalta muodostettiin ohjauksesta QPR ProcessGuide -ohjelman avulla prosessikuvaus.

Opinnäytetyön tuloksista käy ilmi, että sytostaattihoitoa saavan keuhkosyöpäpotilaan ohjausprosessi jakaantuu ohjauksen suunnitteluun, ohjauksen toteutukseen, ohjauksen arviointiin ja jatkohoidon turvaamiseen. Ohjaukseen osallistuvat poliklinikan, Päiväsairaalan ja vuodeosaston sairaanhoitajat ja lääkäri. Tulosten mukaan ohjaukseen osallistuu tarvittaessa myös muita ammattiryhmiä, kuten sosiaalityöntekijä ja ravitsemusterapeutti. Laajan aineiston vuoksi ohjauksesta tuli paljon sisältöjä, joista mainittakoon muun muassa ohjaus sairauden aiheuttamista muutoksista jokapäiväiseen elämään ja lääkehoidon ohjaus. Pääosin sytostaattihoitoa saavan keuhkosyöpäpotilaan ohjausprosessi alkaa poliklinikalta, jonka jälkeen potilas siirtyy Päiväsairaalaan saamaan sytostaattihoitoja, jos lääkäri niitä hänelle määrää. Sytostaattihoitojen jälkeen lääkäri arvioi potilaan vointia ja suunnittelee jatkohoidon. Potilaan jatkohoidossa on useita vaihtoehtoja. Häntä voidaan hoitaa joko kotona, erikoissairaanhoidossa, Terhokodissa tai perusterveydenhuollossa.

A process description of the guidance of a lung cancer patient receiving chemotherapy

The thesis was carried out in the Triangle Hospital project. Triangle Hospital is situated next to Meilahti Tower Hospital on Meilahti campus. It is a new hospital for internal medicine and it was completed in November 2010. The Triangle Hospital Project is a collaboration project between the hospital district of Helsinki and Uusimaa, Department of Medicine at Helsinki University Central Hospital and Otaniemi unit of Laurea University of Applied Sciences.

The objective of the thesis is to develop the guidance of a lung cancer patient receiving chemotherapy in the Clinic for Pulmonary Diseases in Meilahti Triangle Hospital. The personnel can also use the process description of the thesis not only in developing the patient guidance but also in the orientation of new employees. The purpose of the thesis was to create a process description of the guidance of a lung cancer patient receiving chemotherapy for the Clinic of Pulmonary Diseases in Meilahti Triangle Hospital.

The research method used in this thesis was qualitative. The data for the thesis was gathered by using a theme interview, which was carried out as a group interview of three nurses in December 2010. The research questions were: which professional groups participate in the guidance of a lung cancer patient receiving chemotherapy and what is the guidance process of a lung cancer patient receiving chemotherapy like. The interview themes were: the preparation for the guidance of a lung cancer patient receiving chemotherapy, the implementation and contents of the guidance and the evaluation of the guidance. The material gathered from the interview was transcribed and analyzed by using inductive content analysis. Based on the analysis the process description was created with the QPR ProcessGuide –programme.

The findings of the thesis show that the guidance process of a lung cancer patient receiving chemotherapy is divided into the planning of the guidance, the implementation of the guidance, the evaluation of the guidance and the ensuring of the follow-up care. The nurses from the outpatient clinic, day hospital and ward participate in the guidance of the patient as well as the physicians. According to the findings there are also other occupational groups, which participate in the guidance if necessary, like a social worker and a dietician. Due to the extensive data of the thesis there was a lot of material on the guidance, e.g. the guidance of the causes of the illness in everyday life and the guidance of the medication. Mainly the treatment path of a lung cancer patient starts from the outpatient clinic after which the patient is transferred to the day hospital to get chemotherapy, if a physician has prescribed it. After the chemotherapy treatments a physician evaluates the patient’s condition and plans the follow-up care. There are several options in the patient’s follow-up care. He can be treated at home, in specialized care, in a hospice or in primary health care.
