Tiivistelmä
Transitioning to adulthood from childhood is a life process which entails moving to the adult care system. As youth get older, they should adequately prepare to transition from childhood to adulthood. There have been several concerns raised by the adult care providers which they have identified from the patients who have transitioned from Pediatric healthcare. Transition care plays a significant role in ensuring that youth with a chronic condition would have better adult care after the transition.

The purpose of this thesis is to explore the process of transition from youth to adult care and the challenges encountered, and finally the importance of having a transition process at an early age. The questions answered by the thesis is the importance of having a transition process possible challenges that are encountered during transition from youth to adult care?

The number of online databases that were used to obtain relevant data includes, EBSCOhost, PubMed and Science Direct. Keywords and research questions were used to search these articles which resulted in 777 articles and were later subjected to inclusion and exclusion criteria that finally yielded 10 articles which were used to answer research questions. The method that was used to appraise the selected articles is CASP critical appraisal tool while data analysis was based on content analysis.

From the collected data, three main themes were evident; Barriers to Effective Transitions, Access to Adult Care Health Needs and Interaction with the Environment. Regardless of the challenges, there is still an impact made by the healthcare professionals in ensuring an effective transition to adult care. Recommendations were also made for further research. There is a need for further research to be undertaken to help understand the better interventions that are applicable to both the HealthCare practitioners and the parents of youths with the special care needs for a better transition process which are based on evidence.
