”Hoida minua, niin kuin haluaisit itseäsi hoidettavan.” Tehostetun palveluasumisen asukkaan itsemääräämisoikeus hoitajien näkökulmasta

Abstract

Opinnäytetyössä kuvataan asukkaan itsemääräämisoikeuden toteutumista ja tukemista tehostetun palveluasumisen yksikössä hoitajien näkökulmasta. Opinnäytetyön tavoitteena on tukea hoitajia työssään ja ratkaisuissaan asukkaiden itsemääräämisoikeuteen liittyen. Aineisto kerättiin kyselytutkimuksella sähköisesti sekä paperilomakkeella keväällä 2022. Kyselyyn vastasi Kainuun alueen Attendo Oy:n neljän tehostetun palveluasumisen yksikön hoitohenkilökunta.

Tutkimusongelmaksi muodostui: Miten asukkaiden itsemääräämisoikeus toteutuu hoitajien kokemana tehostetun palveluasumisen yksikössä? Millaisia syitä he kuvaavat olevan itsemääräämisoikeuden toteutumattomuuden taustalla?

Työyksiköissä suurin osa hoitajista koki asukkaiden itsemääräämisoikeuden toteutuvan ja hoitajat pystyivät tukemaan asukkaan itsemääräämisoikeutta. Itsemääräämisoikeuden toteuttamisen haasteena koettiin henkilöstön vähyys, asukkaiden huonokuntoisuus, hoivakodin rutiinit sekä asukkaan heikentynyt päätöksentekokyky. Ratkaisuina asukkaiden itsemääräämisoikeuden tukemiseen/lisäämiseen hoitajat toivat esille asukkaiden tuntemisen, henkilöstön lisääminen sekä asukkaat otettaisiin mukaan päätöksen tekoon.

Opinnäytetyön tuloksia voidaan ensisijaisesti hyödyntää asukkaan itsemääräämisoikeuden huomioimisessa ja tukemisessa tehostetun palveluasumisen yksiköissä, joissa tutkimus toteutettiin. Opinnäytetyö voi toimia välineenä keskustelun lisäämiseen hoitajien keskuudessa asukkaiden itsemääräämisoikeuden toteutumisesta sekä rajoittamistoimenpiteiden käytöstä ja niiden ennaltaehkäisemisestä. Opinnäytetyön tulokset mahdollisesti myös lisäävät pohdintaa hoitajien keskuudessa siitä, miten asukkaiden itsemääräämisoikeuden toteutumista voitaisiin lisätä ja kehittää. Myöskin siitä miten rajoittamistoimenpiteitä voitaisiin vähentää tai ennaltaehkäistä. Tuloksia voidaan hyödyntää myös muissa tehostetun palveluasumisen yksiköissä. Tuloksia voivat hyödyntää myös hoitotyön opiskelijat, ammattilaiset ja muutkin aiheesta kiinnostuneet.

Jatkotutkimusaiheena voisi olla asukkaiden Itsemääräämisoikeuden toteutumien tehostetun palveluasumisen yksikössä omaisten/asukkaiden näkökulmasta, rajoitustoimenpiteiden käyttö tehostetun palveluasumisen yksikössä, onko hoitajien koulutuksella tai kokemuksella merkitystä asukkaan itsemääräämisoikeuden toteutumisessa.

This thesis describes the realization and support of the resident's right to self-determination in an enhanced assisted living unit from the nurses’ perspective. The aim of this thesis is to support nurses in their work and solutions related to the residents' right to self-determination. The data was collected both with an electronic survey and paper form in spring 2022. The survey was answered by the nursing staff in four Attendo Oy enhanced assisted living units in Kainuu region. The research problems were the following: how was the residents' right to self-determination realized as experienced by nurses, and what kind of reasons the nurses thought led the non-realization of the residents' right to self-determination? Most nurses in the enhanced assisted living units felt that the residents' right to self-determination was realized and that the nurses were able to support the residents’ right to self-determination. The challenges in realizing the right to self-determination were perceived to be the lack of staff, poor condition of the residents, routines of the living units and reduced decision-making ability of the residents. The nurses highlighted knowing the residents personally, having more staff and involving the residents in the decision-making process as ways how the residents’ right to self-determination could be supported/increased. The results of this thesis can primarily be used to consider and support the residents’ right to self-determination in those enhanced assisted living units where the research was carried out. This thesis can serve as a tool for nurses increasing discussion on the realization of the residents' right to self-determination and the use and prevention of restrictive measures. The results may also increase reflection among nurses on how the realization of residents' right to self-determination could be increased and developed and how the use of restrictive measures could be reduced or prevented. The results can also be utilized in other enhanced assisted living units. The results can also be used by nursing students, professionals and others interested in the topic. Further research topics could be residents' self-determination in an enhanced assisted living unit from the perspective of relatives/residents, the use of restrictive measures in an enhanced assisted living unit, and the relevance of nurses’ education or experience to the realization of residents’ right to self-determination.
