Turvallinen CPAP- ja BIPAP-hoidon hallinta

Abstract

Tiivistelmä

CPAP- (continuos positive airway pressure) ja BIPAP- (bi-level positive airway pressure) hoidolla tarkoitetaan kahta erilaista paineventilaatiotukea. Niiden avulla voidaan hoitaa vaikeasta hengitysvajauksesta kärsivää potilasta, joka hengittää spontaanisti eikä tarvitse intubaatiota. Hoidon onnistumisen kannalta sairaanhoitajalta vaaditaan laiteteknologian hallintaa sekä potilaan tarkkailuun liittyvää osaamista.

Opinnäytetyöni on toiminnallinen, ja sen toimeksiantaja oli Karelia-ammattikorkeakoulu. Opinnäytetyöni tarkoituksena on lisätä hoitotyön opiskelijoiden tietoutta hengitysvajauksesta ja sen hoidosta, sekä parantaa opiskelijoiden laiteosaamista CPAP- ja BIPAP-laitteilla. Opinnäytetyöstäni syntyi tuotoksena Powerpoint-esitys, jossa käsitellään hengitysvajausta ja sen hoitotyötä sekä CPAP- ja BIPAP-laitteita ja niiden käyttämistä havainnollistavilla kuvilla.

Powerpoint-materiaali esitestattiin helmikuussa 2014, jolloin opinnäytetyöntekijä itse luennoi aiheesta 6. lukukauden opiskelijoille. Luennon päätteeksi yleisöltä kerättiin kirjallinen palaute jossa kysyttiin Powerpoint-esityksen hyvistä puolista sekä kehitysideoita. Palautteen pohjalta Powerpointiin tehtiin tarvittavat muutokset. Kehittämismahdollisuudet produktia ajatellen ovat rajattomat. Produktia voisi käyttää myös työelämässä eri yksiköissä perehdytyksen apuvälineenä. Työ voisi osittain toimia myös lähihoitajaopinnoissa. Powerpoint-materiaalia voisi syventää faktojen muuttuessa ja hoitotyön kehittyessä. Aihealuetta voisi myös laajentaa lisäämällä tietoa esimerkiksi uniapnean hoidosta.

Abstract

CPAP (Continuous positive airway pressure) and BIPAP (bi-level positive airway pressure) treatments mean pressure ventilation support. With them we are able to treat patients who suffer from a difficult respiratory insufficiency, but have spontaneous respi-ration and do not need intubation. For the treatment to be successful, the nurses are expected to master the technology of the appliance and the observation of patients.

My thesis is functional and the commissioner is Karelia University of Applied Sciences. The purpose of this work is to upgrade nurse students’ knowledge of the respiratory syndrome and its treatment and, furthermore, to improve students’ CPAP and BIPAP machine skills. The outcome of my thesis was a Power Point presentation in which respiratory insufficiency, its treatment, CPAP and BIPAP machines and their use are introduced with pictures. The Power Point presentation was tested in February 2014, when the author of the thesis lectured about subject to sixth-semester students. In the end of lecture, feedback was collected from the audience. The audience was asked to give feedback on the positive aspects of the presentation and whether there was something to improve. Based on the feedback, improvements were made in the presentation.

Development possibilities based on this thesis are endless. The presentation could be used also in different areas of working life as a device of orientation. The presentation could also partly work in practical nurse studies. The Power Point material could be altered as facts change and nursing develops. The theme area could also be expanded by adding some information, for example about sleep apnea.
