1- ja 2 tyypin diabeetikoiden varvashaavojen hoito-ohje hoitohenkilökunnalle.

Abstract

Opinnäytetyön tarkoituksena oli kehittää ohje Vantaan ja Keravan sairaalapalveluilla työskenteleville terveydenhuollon ammattilaisille ja tarjota ammattilaisille tietoa 1-ja 2 tyypin diabeteksen varvashaavojen ennaltaehkäisystä, tunnistamisesta ja hoidosta helppolukuisen ohjeen avulla. Toimeksiantaja toivoi helppolukuista ohjetta, jota voidaan hyödyntää uusien työntekijöiden perehdytys- ja kehitysmateriaalina.

Ohje toteutettiin A4 julisteena ja sen kehittämisessä käytettiin monipuolisesti suomenkielisiä artikkeleita ja kirjoja aiheesta, sekä aiheeseen liittyviä englanninkielisiä tieteellisiä julkaisuja. Ohjeen toteutuksessa hyödynnetyt lähteet perustuvat tutkittuun ja ajankohtaiseen tietoon diabeettisista varvashaavoista. Lähteiden luotettavuutta arvioitiin objektiivisesti laadullisen tutkimuksen kriteereihin nojaten.

Ohje sisältää viisi aihepiiriä, joihin sisältyvät ennaltaehkäisyn, tunnistamisen ja hoidon pääperiaatteet. Opinnäytetyön teoriaosuudessa käytiin läpi myös kattavasti diabeteksen vaikutus varvashaavojen syntyyn, jalkojen tutkiminen ja haavaluokitus. Ohjeen kehittämisessä hyödynnettiin työelämäkumppanin, sekä Laakson sairaalassa- ja HUS alueella työskentelevien terveydenhuollon ammattilaisten näkökulmia aiheesta. Palaute kerättiin Peijaksen haavahoitoosaston työntekijöiltä, Laakson sairaalan haavahoitajalta, HUS-alueen terveydenhuollon ammattilaisilta ja toimeksiantajalta Vantaan ja Keravan hyvinvointialueelta. Palautteen avulla ohje muokattiin tiiviimmäksi ja varmistettiin vastaamaan sairaalapalveluiden tarpeita.

Jatkokehittämisideana voisi toteuttaa laajemman ja yksityiskohtaisemman ohjeen, joka pitää sisällään tarkemman kuvauksen haavojen laajuudesta ja luokituksesta. Aiheeseen liittyvä työpaja voisi tukea aiheen sisäistämistä. Tutkimusideana voitaisiin selvittää ohjeen vaikutusta varvashaavojen ennaltaehkäisyn ja hoidon käytäntöihin.

The purpose of this thesis was to develop a guide to healthcare professionals working in the Vantaa and Kerava Wellbeing Services County (VAKE) and provide them with information on the prevention, identification and treatment of type 1 and type 2 diabetic toe ulcers through an easy-to-read guide. The client of this thesis had requested an easy-to-read guide that can be utilized as orientation and development material for new employees.

The guide was implemented as an A4 size poster, which was developed using Finnish language articles and books diversly, as well as English language scientific publications. The sources used on this thesis were based on studied and current information of diabetic toe ulcers. The reliability of used sources was assessed objectively based on the criteria of qualitative research.

The guide includes five subject matters that discuss the main principles of prevention, identification and care. The theoretical part of this thesis also comprehensively reviewed the impact of diabetes on diabetic toe ulcers origin, examining the feet and wound classifying. In the development of this thesis, the authors utilized views on the topic from a work life partner, Laakso Hospital professionals and HUS region professionals. The feedback was collected from the employees of the wound care department of Peijas Hospital, the healthcare professionals of HUS region and the Vantaa and Kerava Wellbeing Services County client. With the help of the feedback, the guide was summarized and ensured to meet the needs of the hospital services.

A further development idea could be to develop a more comprehensive and detailed guide that includes a more precise description of the extent and classification of wounds. A workshop related to the topic could support the understanding of it. A research idea could be to investigate the impact of the guide on prevention and treatment practices of toe ulcers.
