Kotisaattohoitotyön toimintaohjeen laatiminen Koskenrinteelle kirjallisina oppaina

Abstract

Tämän opinnäytetyön tarkoituksena oli selvittää, mitä on muistisairaan kokonaisvaltai-nen saattohoitotyö, sekä luoda Palvelutaloyhdistys KOSKENRINNE ry:lle uusi koti-saattohoitotyön toimintaohjeistus uusimman näyttöön perustuvan tiedon pohjalta. Toimintaohjeet on tuotettu kahtena oppaana muodossa, joista molemmat tulevat käyt-töön Koskenrinteen hoivaosastojen hoitohenkilökunnan päivittäisen saattohoitotyön tueksi. Toinen opas on hoitohenkilökunnalle tarkoitettu saattohoitotyön ohjeistus, jo-hon on sisällytetty jo aiemmin Koskenrinteellä käytössä ollut saattohoidon tarkistuslis-ta. Toinen opas on tarkoitettu annettavaksi vainajan omaiselle, ja se tarjoaa saajalleen tukea surun keskelle sekä käytännönläheisiä ohjeita kuoleman jälkeisistä toimenpiteistä.

Opinnäytetyö on toteutettu kehittämistyönä toimintatutkimuksen muodossa. Työssä esitellään yksityiskohtaisesti toimintatutkimuksen määritelmä, rakenne sekä tutkimus-prosessi ja niiden mukaisesti myös opinnäytetyön tekovaiheet. Työhön sisältyy alkuti-lan kartoitus, jonka pohjalta Koskenrinteen toimintaa lähdettiin kehittämään sekä laaja uusimpaan näyttöön perustuvaan tietoon perustuva kirjallisuuskatsaus, jonka pohjalta oppaat ovat laadittu tiiviissä yhteistyössä työelämän edustajien kanssa Koskenrinteen tarpeita palveleviksi kokonaisuuksiksi. Koskenrinteen saattohoitoa tarvitsevat asukkaat ovat pääosin ikääntyneitä muistisairaita, joten saattohoitotyötä on keskitytty tarkaste-lemaan heidän tarpeitaan vastaavasti.

Terminal care (TMC) is a demanding, both physically and professionally challenging working area in nursing. In Finland we have had a new nationwide guidance for ter-minal care since 2010, but there are still some facilities giving terminal care without written guidance or proper knowledge about how to produce it decently.

The objective of this Thesis is to explore what all-inclusive terminal care for a person suffering of memory disorder is and to produce a written guidance about that subject for sheltered housing association Koskenrinne to improve their work in TMC area. The guidance includes two separate pieces: one for nurses to use in daily work and the oth-er to give for the relatives of a dead resident. This Thesis is a development work in a form of action research. The research was produced by interviewing the personnel of Koskenrinne about their current state in TMC area and by an extensive national and in-ternational TMC literature review. The guidance is based on the review of the litera-ture.

According to our research there is till lack of written guidance about how to meet and treat a dying person with memory disorder. The produced guidance is going to be used in every department of Koskenrinne as a part of daily work. The purpose of the guid-ance is to give nurses valuable information about TMC and to help them to offer high quality terminal care for the residents of Koskenrinne.
