Hoitajien osaaminen Clostridium difficile -infektioon sairastuneiden potilaiden hoitotyössä

Abstract

Tämän opinnäytetyön tarkoituksena oli kartoittaa Porin perusturvan, kuntoutus-ja sairaalapalveluiden akuutti lyhytaikaishoidon osastolla T4 työskentelevien hoitajien tiedot, osaaminen sekä valmiudet Clostridium difficile -infektioon sairastuneiden potilaiden hoitotyössä sekä Clostridium difficile -infektion torjunnassa. Tavoitteina oli tiedon saanti, osaamisen syventäminen sekä hoitotyön keinojen kehittäminen Clostridium difficile -infektion uusiutumisen ehkäisemiseksi sekä infektion torjumiseksi.

Tutkimusmenetelmä oli kvantitatiivinen survey -tutkimus. Tutkimusaineisto kerättiin syksyllä 2016 kyselylomakkeella ja kohderyhmän muodostivat osastolla T4 työskentelevät sairaanhoitajat sekä perus/lähihoitajat. Kyselylomakkeessa oli 50 suljettua ja monivalintakysymystä sekä kaksi avointa kysymystä. Kyselyllä selvitettiin vastaajien taustatiedot, hoitotyön kokemus ja tiedot Clostridium difficile -bakteerista, -infektiosta ja -infektion torjunnasta sekä tiedonsaanti koskien Clostridium difficileä.

Tämä tutkimus analysoitiin tilastollisia menetelmiä käyttäen. Tutkimustulokset esitettiin lukumäärinä ja pieni osa prosentteina. Tulokset selitettiin sanallisesti sekä osa tuloksista taulukoitiin havainnollistamaan tutkimustuloksia. Avoimet kysymykset analysoitiin luokittelemalla samankaltaiset vastaukset ryhmiin ja tulosten perusteella tehtiin yhteenveto.

Tulosten mukaan kyselyyn vastanneilla sairaanhoitajilla sekä perus/lähihoitajilla on hyvä osaaminen kosketusvarotoimien noudattamisessa, käsihygienian toteuttamisessa sekä infektion torjuntaan vaikuttavista hoitotyön menetelmistä. Heikommin oli tietoa Clostridium difficile -bakteerista ja -itiöiden säilymisestä pinnoilla viikkoa pidemmän ajan. Clostridium difficilen yleiset oireet hoitajat tiesivät hyvin, mutta muita oireita tiedettiin heikommin. Tiedonsaantia koskevien kysymysten tuloksissa oli hajontaa. Osa vastaajista koki saavansa tietoa erittäin paljon ja osa koki tiedonsaannin puuttuvan kokonaan.

Jatkossa kehittämiskohteena osastolla T4 voisi olla kosketusvarotoimien tehostaminen sekä potilaiden ja omaisten ohjaaminen Clostridium difficile -infektioiden uusiutumisen ja leviämisen ehkäisemiseksi sekä tiedonsaantia koskevien ohjeiden päivittäminen. Jatkotutkimushaasteena voisi toteuttaa tutkimuksen, jonka avulla selvitettäisiin Clostridium difficile -infektioon sairastuneen potilaan ohjausta sekä sairaalahoidon aikana, että kotiutusvaiheessa. Mielenkiintoista olisi selvittää asiaa sekä potilaiden, että hoitajien näkökulmasta.

The purpose of this thesis was to determine the familiarity, skills and knowledge of the nurses, working on rehabilitation and respite ward T4 in Pori, about C. difficile patients’ treatment and the prevention of the disease. The aim was to gather information, broaden knowledge and to improve protocol to prevent C. difficile- infection spread and its recurrence.

This was a quantitative survey. Data were collected with a questionnaire in 2016. Target group consisted of the RN’s and practical nurses working on the ward T4 of the hospital. The questionnaire comprised 50 closed-ended and multiple-choice questions and two open-ended questions. The questionnaire incorporated questions on nurses’ demographic data, their knowledge about C. difficile-bacterium and – infections, prevention of infections and nurses’ main source of information regarding the disease.

This study was analyzed statistically. Results of the study were mainly presented as numbers, a few findings were shown as percentages. The findings were explained in text-form and some of the numerical data were also illustrated with tables for clarity. Open questions were analyzed by using qualitative content analysis.

The results show that both RN’s and practical nurses possess accurate knowledge of contact precautions, implementing hand hygiene and about the protocols affecting the spread of infection. However, there are still areas where the knowledge was lacking. For example, nurses were unaware of the fact that spores produced by the C. difficile bacterium can live on environmental surfaces for more than a week. Nurses were knowledgeable of the most common C. difficile infection symptoms, but the results suggested that there is a knowledge deficit in identifying some of the associated symptoms of infection. There was considerable dispersion in the answers concerning education and training. Some of the respondents reported receiving a lot of education, whereas others considered it to be either insufficient or non-existent.

The results of this study revealed a few clear targets for development, such as, the implementation of C. difficile contact precautions, further enhancement of the information flow to the patients and their family members on how to prevent infections and their recurrence and, finally, updating the guidelines concerning the education of the staff. Future studies could examine C. difficile patient’s counselling during hospitalization and on discharge. Addressing the topic from both nurses’ and patients’ perspective could yield interesting information.
