Sähköisen rakenteisen kirjaamisen kehittämisprosessi Kainuun keskussairaalan lastenpsykiatrian osastolla

Abstract

Projektin tarkoituksena oli kehittää sähköistä rakenteista kirjaamista, FinCC -luokitusten käyttöä, Kainuun keskussairaalan lastenpsykiatrian osastolla. Tavoitteena oli luoda uusi kirjaamisen mallinnus. Metodologiana oli kehittävä työntutkimus ja johtajuuden näkökulmana valmentava johtajuus, joissa korostetaan yksilön ja yhteisön omaa luovuutta ja reflektointikykyä. Projektissa yhteistoiminnallisuus vastuutti mukanaolijoita kehittämistyöhön. Motivaatiota ja sitoutumista lisää yleisesti se, kun henkilökunta pystyy itse vaikuttamaan te-kemäänsä työhön.

Kirjaamisen kehittäminen lähti tarpeesta. Sähköinen rakenteinen kirjaaminen oli kaikille työntekijöille uutta ja tietoa rakenteisen luokituksen käytöstä tarvittiin. Projektissa käytiin läpi kehittävän työntutkimuksen syklin kol-me ensimmäistä vaihetta. Osaston kirjaamisen toimintajärjestelmän muodostivat hoitotyöntekijät, sähköinen potilasasiakirja, tulosyksikkö, kirjaamista säätelevät lait, hoitotyöntekijät kirjaavat, FinnCC –luokituksen käyttö ja uusi mallinnus. Alkutilanteen kartoitus tehtiin haastattelemalla kirjaamiseen perehtynyttä apulaisosastonhoitajaa ja ensimmäiseksi työtä hankaloittavaksi tekijäksi nousi vaikeaksi koettu sähköisen potilasasiakirjan käyttö. Tässä projektissa ei voitu kehittää valtakunnallista teknistä järjestelmää. Seuraavaksi tarkasteltiin kirjaamisen historiaa toiminnan kehityskulun ja senhetkisen toimintatavan ristiriitojen löytämiseksi. Huomattiin, että manuaalisessa kirjaamisessa hoitotyön prosessimalli oli käytössä, sähköisestä kirjaamisesta se puuttui ja rakenteinen kirjaaminen toi jälleen hoitotyön näkyviin prosessimallia noudattelevana. Henkilökunta pohti omia kehittämistarpeitaan ja sopi yhteisesti kehittämismenetelmistä. Rakenteisen luokituksen käytön kehittäminen nousi tämän projektin kehittämiskohteeksi. Koulutus ja projektiryhmän laatima kirjaamisen mallinnus olivat ratkaisuja uutta luovassa ekspansiivisen oppimisen prosessissa.

Työntekijät arvioivat, että kehittämisprosessi ja siinä luotu mallinnus toivat kirjaamiseen selkeyttä, yhtenäisyyttä, johdonmukaisuutta ja ajansäästöä. Kirjaaminen helpottui ja luokitusten käyttö monipuolistui. Hoitotyön päätöksentekoprosessi toi hoitajan työtä näkyväksi. Työyhteisölle prosessi toi yhteisten sopimusten ja sääntöjen kautta varmuutta ja turvallisuutta kirjaamiseen.

The purpose of the project was to improve digital structural documentation, the use of the FinCC classification, in the psychiatric ward for children at Kainuu Central Hospital. The aim was to create a new written procedure for documentation and to do it with the methodology of developmental work research and coaching leadership. The methodology of developmental work research and coaching leadership emphasizes both the individual’s and the community’s creativity and ability to reflect. Collaboration in the project gave the staff responsibility for the development work. Generally, motivation and commitment increase when the staff can influence its own work.

The documentation needed to be improved. The digital structural documentation was new to all of the staff and they needed knowledge of how to use the structural classification. The three phases of the cycle of developmental work research were reviewed. There were many factors of the activity system of developmental work research in the ward’s documentation. These were the nursing staff, digital patient records, profit centre, laws regulating documentation, staff doing the documentation, use of the FinnCC classification and the new written procedure for documentation. The deputy head nurse, who had experience of digital documentation, was interviewed to survey the circumstances in the beginning. The first issue was that the use of digital patient records was experienced to be difficult. The national technical system could not be developed in this project. Next, the history of documentation was studied to find contradictions between the process and the current procedure. It was noticed that the nursing process approach was used in manual documentation but not in digital documentation. The nursing process approach was reintroduced in structural documentation. The staff reflected on their own needs for development and together agreed on the methods of development. The improvement of the implementation structural classification emerged as the object for development in this project. Education and the written procedure for documentation by the participants were the answers in the creative expansive learning process.

According to the staff , the development process and the written procedure of the development made documentation clearer, more consistent, coherent and also time-saving. Documentation was easier and use of the classification more versatile. The decision-making process in nursing made nurses’ work more visible. The joint agreements and rules brought reliability and safety to documentation and thus helped the work community.
