Tiivistelmä
Tämän kehittämisprojektin tavoitteena oli parantaa ja yhtenäistää aikuisvastaanotolla toteutettavaa diabetesasiakkaiden omahoidon tukea. Projekti oli työn tilaajan toive, koska diabetesasiakkaiden määrä on kasvanut ja on löydettävä uusia toimintatapoja heidän hoitonsa tukemiseksi. Projektin tarkoituksena oli selvittää asiakkaiden näkökulmasta terveystarkastuksen merkitykset, sekä niistä tuloksista jatkaa terveystarkastuksien ohjaustavan kehittämistä omassa organisaatiossa. Projekti eteni suunnitelman mukaisesti.
Projektiin liittyvän tutkimuksen tarkoituksena oli selvittää diabetesasiakkaiden näkökulmasta terveystarkastuksien merkitys Salon terveyspalveluissa eli se, onko Salon terveyskeskuksen diabeteshoitajan vastaanotolla tehtävä terveystarkastus asiakkaan hoitoa ja omahoitoa tukeva. Tutkimus toteutettiin keväällä 2018. Aineiston keruu tapahtui strukturoidulla kyselylomakkeella, joka jaettiin Salon terveyskeskuksessa diabeteshoitajan vastaanotolla käyneille tyypin 2 diabetesasiakkaille jokaisella terveysasemalla (N=150). Kyselyyn vastasi 96 asiakasta vastausprosentin ollessa 70%. Tutkimustulosten mukaan lähes kaikki vastaajat olivat tyytyväisiä diabeteshoitajan vastaanottokäynteihin ja tämän antamaan ohjantaan. Tutkimuksen perusteella kuitenkin ilmeni, että tarvittaisiin lisää tukea elintapojen muuttamiseen, kuten vertaistukiryhmät sekä sähköinen yhteydenpitokanava hoitajille.
Tutkimustulosten pohjalta kehitettiin terveystarkastuskäynnin toteutustapaa muun muassa siten, että projektiryhmän ensin tekemän pilotointijakson jälkeen otettiin Terveyskylän Diabetestalo mukaan asiakkaalle annettavaan ohjaukseen. Kehittämistoimenpiteet jatkuvat vielä osittain tämän projektin päättymisen jälkeenkin ja kaikki kehitetyt asiat on tarkoitus ottaa käyttöön Salon kaikissa aikuisvastaanoton toimipisteissä. The aim of the development project was to improve and harmonize the support of diabetes clients in adult education. The project was the wish of the job subscriber because the number of diabetes clients has increased, and new approaches must be found to support their care. The purpose of the project was to determine the meanings of the health check from the customer's point of view and to continue to develop the health control approach within its own organization.
The project progressed according to plan. The purpose of the project-related study was to investigate the importance of health controls for diabetes clients in Salo Health Services, i.e. whether a health check is carried out at the Health Centre of the Salo in the diabetes customer care and self-care support. The study was conducted in spring 2018. The data collection took place in a structured questionnaire, which was distributed at the Salon Health Centre for type 2 diabetes clients at each health clinic (N = 150). The survey replied 96 customers with a response rate of 70%. According to the survey results, almost all respondents were satisfied with the reception visits of the diabetes nurse and his control. However, the investigation showed that there was a need for further support for changing lifestyles, such as peer-group groups and electronic contact channel for medical staff.
Based on the results of the study, the implementation of the health inspection visit was developed, inter alia by the inclusion of the Terveyskylä Diabetestalo in the guidance for the customer after the pilot phase of the project team. Development measures are still in part after the completion of this project and all the issues developed are to be deployed at the Salo in all adult reception locations.
