Mini-Mental State Examination testin tekemisen ohjeistaminen kotihoidon työntekijöille Tampereen kaupungilla

Abstract

Muistisairauksien yhteiskunnalliset kustannukset ovat suuret ja ennusteiden mukaan muistisairaiden määrä kasvaa tulevaisuudessa räjähdysmäisesti. Tämän vuoksi hoitajien osaaminen testaamisessa on ensiarvoisen tärkeää, jotta saadaan tasalaatuisella testaamisella oikeanlaisia tuloksia. Opinnäytetyön tarkoituksena oli selvittää, miten Tampereen kaupungin kotihoidon työntekijät on ohjeistettu Mini-Mental State Examination (MMSE) testin tekemiseen asiakkaille. Ta-voitteena on tuottaa tietoa kotihoidon esimiehille ja palvelupäälliköille työntekijöiden kokemukseen perustuen heidän saamastaan testin tekemisen ohjeistamisesta.

Kohdejoukoksi valikoitui kotihoidossa työskentelevät lähihoitajat, koska yleensä he teettävät MMSE-testin asiakkaalla. Opinnäytetyö toteutettiin kvalitatiivisena tutkimuksena, jossa aineisto kerättiin teemahaastatteluilla. Haastatteluaineisto litteroitiin ja analysoitiin sisällönanalyysillä.

Tulosten mukaan kotihoidon työntekijät kokevat, että työnantaja ei ole ohjeistanut heitä MMSE-testin tekemiseen. Testin tekeminen oli kirjavaa ja osin virheellistä, jolloin muistitestin tulokset eivät vastaa asiakkaan oikeaa muistin tilaa. Kaikki hoitajat olivat yhtä mieltä siitä, että he tarvitsevat kunnon perehdytyksen testin tekemiseen.

Opinnäytetyön johtopäätöksenä voidaan todeta olemassa oleva riski, jossa Tampereen kaupungin kotihoidon asiakkaat eivät saa tällä hetkellä asianmukaista eivätkä tasalaatuista muistin testausta, koska työntekijöiden kokemuksen mukaan työnantaja ei ole varmentanut hoitajien osaamista testin tekemiseen. Jonka vuoksi asiakkaiden muistisairauden hoidon alkaminen saattaa jopa viivästyä. Esimiehet ja palvelupäälliköt voivat halutessaan hyödyntää opinnäytetyön tuloksia järjestämällä hoitajille koulutusta testin tekemiseen. Jatkotutkimusaiheena voisi olla MMSE-testin ohjeistamiskoulutuksen suunnittelu, kokeilu, toteutus sekä kirjalliset ohjeet siihen.

The socially costs of memory diseases are high and, according to forecasts, the number of memory patients will increase exponentially in the future. This is why nursing skills in testing are of the highest importance, providing consistent results with consistent testing. The purpose of this thesis was to find out how the city of Tampere home care workers have been instructed to do a Mini-Mental State Examination (MMSE) test for clients. The priority was to provide information to home care service supervisors and service managers about employee’s experience based on the test guideline that they received.

The home care service practical nurses were chosen as the target group, as they usually test the MMSE on the client. The thesis was carried out as a qualitative study, in which the material was collected by focused interviews. The interview material was transcribed and analyzed by content analysis.

According to the results, home care workers feel that the employer has not instructed them to do the MMSE test. The test was varied and partly inaccurate, so the results of the memory test do not match the client's real memory space. All practical nurses agreed that they needed a proper orientation for a test.

The conclusion of the thesis is that there is an existing risk where home care clients in the City of Tampere do not currently receive proper and consistent testing of memory, as the employees' experience shows that the employer has not ensured the skills of the caregivers to do the test. That is why it may even delay the start of treatment for clients' memory disease. Home care service supervisors and service managers can, if they wish, utilize the results of the thesis by arranging training for practical nurses to perform the test. The topic of further research could be the planning, experimentation, implementation and written instructions of the MMSE test guidance training.
