Tiivistelmä
Tämän opinnäytetyön tarkoituksena oli kehittää strukturoitu kyselylomake toimeksiantajana toimivalle Varsinais-Suomen Sairaanhoitopiirin (VSSHP) Turun yliopistollisen keskussairaalan (Tyks) Sydänkeskukselle vuoronvaihtoraportointimenetelmän vaihtumisen vaikutusten arviointiin. Opinnäytetyö toteutettiin narratiivista kirjallisuuskatsausta (n=22) mukaillen. Tavoitteena oli kehittää VSSHP:n yhden osaston vuoronvaihtoraportoinnin sujuvuutta ja toimivuutta.
Työssä kartoitettiin aineistoissa esiintyneisiin vuoronvaihtoraportointimenetelmiin. Suullisessa raportoinnissa yksi tai useampi hoitaja kertoo suullisen yhteenvedon vuorossa tapahtuneista asioista ja potilaiden tilanteesta. Tämän raportoinnin tyylin vahvuuksia ovat mahdollisuus kysyä tarkentavia kysymyksiä raportin aikana ja sen positiivinen vaikutus työyhteisön ilmapiiriin. Sen heikkouksia on raportointiin kuluvan ajan määrä ja lisääntynyt raporttiin liittymätön keskustelu. Hiljaisessa raportoinnissa hoitajat lukevat omatoimisesti hoitokertomuksesta edellisen vuoron tapahtumat ja potilaiden tilanteet. Sen vahvuuksia ovat raportointitilanteen riippumattomuus edellisen vuoron työntekijöiden käytettävyydestä ja raportointiajan lyheneminen. Sen heikkoudet ovat tarkentavien kysymysten kysymisen vaikeus ja raportin laadun riippuvuus kirjaamisen laadusta. Vuoteenvierusraportoinnissa raportointi tapahtuu potilaiden vuoteiden vieressä, jossa potilaiden tilanteet käydään läpi kunkin potilaan läsnä ollessa. Tämän vahvuutena on mahdollisuus ottaa potilas osaksi tämän hoitotyön suunnittelua ja toteutusta, sekä tutkia potilaan tilaa fyysisesti samalla, kun kuuntelee raporttia ja sen heikkouksia ovat yksityisyyden turvaamisen vaikeus ja häiriöt tilanteessa.
Aineiston analyysin perusteella kehitettiin strukturoitu kyselylomake, jolla voidaan arvioida vuoronvaihtoraportointimenetelmän vaihtamisen vaikutusta raportoinnin laatuun ja sen vaikutuksiin potilasturvallisuuteen, hoitotyön kirjaamisen laatuun ja työyhteisön ilmapiiriin. Kyselylomakkeessa taustamuuttujina ovat ikä, työkokemus (hoitoalalla ja nykyisellä osastolla) ja ammattinimike. Kysymyksiä on 15, joista 13 on likert-asteikolla ja 2 on avoimia kysymyksiä. Toimeksiantaja toteuttaa kyselyn myöhemmin. The purpose of this thesis was to develop a structured questionnaire for the commissioner Heart Centre of Turku University Hospital of the Hospital District of Southwest Finland to evaluate the effects of changing change of shift report methods. This thesis was produced using a narrative literature review (n=22). The goal of this project was to further develop the fluency and efficacy of change of shift reporting for said ward.
In the thesis the importance of documentation regarding the flow of information in a hospital organisation was examined along with its importance in change of shift reporting. It's especially important as a part of reading handovers. In the thesis all of the reporting methods used by the Hospital District of Southwest Finland were examined. In verbal reports one or more nurses tell a verbal summary of the events of the previous shift and the conditions of the patients. The strengths of this style of reporting are the possibility of asking for more details and the positive effect it has on the atmosphere of the work environment. The weaknesses are the amount of time spent on the reporting and increased talk unrelated to the report. In silent reporting nurses of the on-coming shift read the events and conditions of the patients from the electronic documentations written by the previous shifts nursing staff. Its strengths are the ability to conduct reporting independent of the availability of the nursing staff of the previous shift and shortening of the time spent during the report. The weaknesses of it are difficulty of asking for more details and the quality of the report being so tied to the quality of documentation. With bedside reporting the report happens at the patients bedside where the condition of each patient is reviewed in their presence. The strengths of this type of reporting are the ability to involve the patient in planning and conducting their care and examining the physical condition of the patient while listening to the report. The weaknesses are the difficulty of maintaining privacy for the patient and the increased interruptions in the reporting situation.

The background variables are: age, work experience (in healthcare and in the current ward) and occupational title. The questionnaire has 15 questions of which 13 are on the likert-scale and 2 are open questions. The commissioner will conduct the questionnaire at a later date.
