Tiivistelmä
Tässä opinnäytetyössä sepelvaltimotautipotilaalla tarkoitetaan sellaista potilasta, jolla sairaus on jo diagnosoitu, ja jonka oireet vaativat jatkotutkimuksia. Opinnäytetyön tarkoituksena oli kartoittaa julkaisuja sepelvaltimotautipotilaan hoitopolusta ja kuvata hoitopolun eri vaiheita siirryttäessä avohoidosta sairaalaan ja takaisin. Tavoitteena on korostaa hoidon sujuvuutta ja ymmärrettävyyttä sekä antaa sairaanhoitajille konkreettinen työväline hoitopolun eri vaiheista potilasohjauksen tueksi. Opinnäytetyön toimeksiantaja oli Varsinais-Suomen sairaanhoitopiiri.
Työ toteutettiin narratiivisena kirjallisuuskatsauksena. Tietopohjana toimivat sosiaali- ja terveysalan julkaisut sepelvaltimotaudista ja sepelvaltimotaudin hoidon eri vaiheista (n=51).
Opinnäytetyön tietokantahaussa käytetyillä hakukriteereillä hoitopolun kokonaisuudesta ei löytynyt kuin muutamia suomalaisia julkaisuja. Hakukriteereillä kansainvälisiä julkaisuja hoitopolun kokonaisuudesta ei löytynyt yhtään. Sen sijaan eri vaiheista yksittäisesti löytyi laajasti tutkittua tietoa, sekä Suomesta että kansainvälisesti. Tietokantahakua täydennettiin manuaalisella haulla. Potilaan polku avohoidosta sairaalaan alkaa oireiden pahenemisesta ja sepelvaltimotautikohtauksesta, Polku jatkuu ensihoidon kautta sairaalaan tutkimuksiin ja hoidon tarpeen arviointiin. Hoidon tarpeen mukaan määräytyy hoitolinja konservatiivisen ja invasiivisen välillä. Lopulta potilas siirtyy takaisin avohoidon piiriin, jossa korostuvat seuranta, elintavat ja lääkitys sekä kuntoutus.
Hoitopolkua kokonaisuutena tulisi tutkia lisää. Hoitopolun kokonaisuuden kuvaaminen hyödyttää terveysalan ammattilaisia, jotka ovat mukana potilaan hoidon suunnittelussa ja toteuttamisessa. Potilaalle hoitopolun mahdollisten vaiheiden ymmärtäminen tuo turvallisuuden tunnetta sairauden kanssa elämiseen sekä varmuutta ja motivaatiota sen hoitamiseen. In this thesis coronary artery disease patient refers to a patient whom the illness has already been diagnosed, and whose symptoms require further examination. The purpose of this thesis was to chart publications considering coronary artery disease patient's care path and to bring out the different phases of the care path when being transfered from outpatient care to hospital care and back. The objective is to emphasise the effortlessness and clarity of care and to give nurses a tangible tool about the different phases of the care path to support their patient education. The principal of the thesis was the hospital district of Southwest Finland.
The work was carried out as a narrative literature review. The thesis was based on publications of social and health services about coronary artery disease and the different phases of coronary artery disease treatment (n=51).
With the search criterias used in the database search of the thesis, only a few Finnish publications were found considering the care path as a whole. International publications considering the care path were not found with the search criterias. Instead, researched information were found widely about the different phases of the care path individually, both from Finland and internationally. Database search was supplemented with manual search. Patient’s path from outpatient care to hospital care begins with a flare-up of symptoms and acute coronary syndrome. The path continues through emergency care to hospital for examinations and evaluation of the need for treatment. The treatment line is determined between conservative and invasive depending on the need for treatment. Eventually, the patient is transfered back to outpatient care, where follow-up, ways of life and medication as well as rehabilitation are emphasized.
The care path as a whole should be further researched. Describing the care path as whole benefits healthcare professionals, who are involved in the planning and implementation of patient care. For the patient, understanding the possible phases of the care path provides safetiness to live with the illness, as well as certainty and motivation to treat it.
