Perushoidon laadun kriteerit poliklinikkahoitotyöhön Varsinais-Suomen sairaanhoitopiirissä – Steppi-hanke

Abstract

Perushoidon oikeanlainen toteutuminen on perusta laadukkaaseen hoitotyöhön. Laadukas perushoito tulee toteuttaa hoitoyksiköstä riippumatta. On tärkeää, että hoitoyksiköissä on yhteiset hoitokäytännöt, joilla turvataan potilaan hyvä hoito.

Tämän opinnäytetyön tarkoituksena oli modifioida Varsinais-Suomen sairaanhoitopiirin perushoidon laadun kriteerit polikliiniseen hoitotyöhön soveltaen yhdelle toimialueelle jo tehtyjä kriteereitä. Tavoitteena oli koota yhteen hyvän perushoidon ydinasiat ja yhtenäistää poliklinikan hoitokäytäntöjä. Opinnäytetyö toteutettiin narratiivisena kirjallisuuskatsauksena (n=48). Toimeksiantajana toimii Varsinais-Suomen sairaanhoitopiiri.

Opinnäytetyön tuloksena kehitettiin posteri hoitohenkilökunnan käyttöön. Polikliiniseen hoitotyöhön kehitetystä posterista löytyy kaikki perushoidon osa-alueet, jotka kuuluvat STEPPI- kriteereihin. Perushoidon osa-alueet, jotka kuuluvat STEPPI-kriteereihin ovat aseptiikka, erittäminen, hengitys ja verenkierto, ihon kunto ja hygienia, kivunhoito, liikunta ja asentohoito, nukkuminen ja lepo, pahoinvointi ja ravitsemus, suunhoito sekä tunteet. Perushoidon osa-alueista on koottu posteriin ydinasiat, jotka tulee ottaa huomioon polikliinisessä hoitotyössä laadukkaan hoidon saavuttamiseksi.

Jatkossa olisi hyvä selvittää tehtyjen kriteerien toimivuutta ja toteutumista poliklinikalla, jotta toimintaa pystytään tulevaisuudessa kehittämään oikeaan suuntaan. Lisäksi olisi hyvä harkita puuttuvien osa-alueiden lisäämistä toimeksiantajan toimialueen vuodeosastolle tehtyihin kriteereihin, sekä selvittää hoitajien mielipiteitä kriteereistä ja kartoittaa mahdollisen lisäkoulutuksen tarve.

The right kind of basic care is a foundation to the high-quality nursing. High-quality basic care should be performed regardless of the nursing unit. It is important that the nursing units have common management practices to ensure good patient care.

The purpose of this thesis was to modify the STEPPI-criteria for outpatient nursing care. The aim was to collect the core matters of the good basic care together and unify the care practices of the outpatient unit. The thesis was carried out as a narrative literature review (n = 48). The databases used by the Turku University of Applied Sciences and the manual search were used to search for publications. The hospital district of Southwest Finland serves as the principal.

As a result of the thesis, a poster was created for use by the nurses of the hospital district of Southwest Finland. In the poster there are all the sectors of the basic care which belong to the STEPPI-criteria. Sectors of the basic care the ones belonging to the STEPPI-criteria are aseptic, secretion, respiratory and blood circulation, skin condition and hygiene, pain management, exercise and posture, sleeping and rest, nausea and nutrition, oral care, and emotions. The core matters which have to be taken into consideration in the outpatient care to reach high-quality care have been composed of the sectors of the basic care to the poster.

In the future, it would be helpful to clarify the functioning and implementation of the criteria in the nursing units in order to develop the activity in the right direction in the future. Furthermore, it would be essential to add the missing sectors to the criteria that have been done to the principal’s unit’s inpatient ward as well as to clarify the opinions of the nurses’ on the criteria and to identify the need for possible additional education.
