Tiivistelmä
Lonkkamurtumapotilaan hoito on vuosittain merkittävä summa maksettavaksi yhteiskunnalle. Arvion mukaan vuodessa yhden lonkkamurtumapotilaan hoito tulee maksamaan noin 30 000 €. Lonkkamurtuma lisää myös riskiä kuolleisuuteen, joten hoitoon ja kuntoutukseen tulee kiinnittää huomiota. Tärkeää on ennaltaehkäisevä työ, jotta kaatumista ei pääsisi tapahtumaan, mutta lonkkamurtuman jälkeen tärkeää on sekundaaristen kaatumisten ehkäisy.
Vanhusten määrä tulee tulevaisuudessa lisääntymään ja sitä mukaa myös kaatumiset lisääntyvät. Keski-Suomessa lonkkamurtumaleikkauksia tehdään paljon ja hoitoprosessiin ja hoidon laatuun tuleekin kiinnittää huomiota.
Keski-Suomeen valmistuu uusi sairaala NOVA vuonna 2020, jolloin lonkkamurtumapotilaiden hoito siirtyy Päivystys- ja infektio-osaston tehtäväksi. Opinnäytetyön tarkoituksena on kuvata, mitä tietoa lonkkamurtumapotilaan hoidossa tarvitaan. Laadukkaan hoidon edellytys on tutkittu, näyttöön perustuva tieto, jonka pohjalta lonkkamurtumapotilaita hoidetaan. Tavoitteena on tuotetun tiedon avulla saada päivystys- ja infektio-osaston hoitajille tietoa lonkkamurtumapotilaan hoidosta ja vahvistaa heidän osaamistaan.
Opinnäytetyö tehtiin haastattelemalla hoitajia, jotka hoitavat tällä hetkellä lonkkamurtumapotilaita. Haastattelu tehtiin teemahaastatteluna ja haastatteluun osallistui neljä (n=4) hoitajaa. Tutkimustuloksista käy ilmi, että lonkkamurtumapotilaan hoidossa tarvitaan laaja-alaista osaamista. Induktiivisen sisällönanalyysin perusteella saatiin kuusi pääluokkaa, jotka ovat: kliininen osaaminen, terveyden edistäminen, eettinen osaaminen ja asiakaslähtöisyys, kirjaaminen ja informointi, kommunikointi ja moniammatillisuus sekä hoidon osaamisen varmistaminen. Tämän lisäksi yhdistäväksi tekijäksi nousi näyttöön perustuva toiminta.
Tutkimustuloksia voidaan hyödyntää lonkkamurtumapotilaan hoitoprosessin kehittämisessä sekä päivystys- ja infektio-osaston hoitajien osaamisen vahvistamisessa. The treatment of hip fracture patients makes up significant amount to be paid by society each year. It has been estimated that the cost of treating one hip fracture patient will be approximately 30,000 € per year. Hip fracture also increases the risk of mortality, so treatment and rehabilitation should be given attention. Preventive work is important. It is very important to prevent falls. After hip fracture, preventing secondary falls is important.
The number of elderly people will increase in the future and because of that, the number of falls will increase. There are many hip fracture operations in central Finland and attention should be paid to the treatment process and the quality of the care.
The new hospital NOVA in Central Finland will be completed in 2020 and then the care of hip fracture patients will be transferred to the Emergency and Infection Department. The purpose of this thesis was to provide information on the treatment of a hip fracture patient and to bring the information to the nurses of the emergency and infection ward for quality treatment of the hip fracture patients.
The thesis was done by interviewing nurses who are currently treating patients with hip fractures. The interview was conducted as a theme interview and four (n = 4) nurses participated on the interview. The results show that extensive knowledge is needed in the treatment of hip fracture patients. Based on inductive content analysis, six main categories were obtained: clinical competence, health promotion, ethical competence and customer orientation, recording and informing, communication and multi-professional care, and ensuring treatment expertise. In addition, evidence-based activities became the unifying factor.
The results of the research can be used to develop the treatment process for the hip fracture patients and to strengthen the skills of the nurses in the nursing and infection department.
