Vastatakseen väestörakenteen muutoksesta aiheutuviin resurssi- ja kustannuspaineisiin, terveydenhuollon toiminnan kehittämisessä asiakkaiden roolia olisi lisättävä. Terveydenhuollon lean-kehittämisellä on haettu ratkaisuja kustannus- ja resurssihaasteisiin, mutta tämänkin ajattelumallin mukaisessa toiminnan kehittämisessä on kohdattu puutteita asiakasosallisuudessa ja asiakkaan osallisuutta mahdollistavissa menetelmissä.
Kehittämisprojektin tarkoituksena oli kehittää asiakaskäyttöön soveltuva mittari hukkatekijöiden tunnistamiseen Uudenkaupungin sosiaali- ja terveyskeskuksen lääkärien ja hoitajien vastaanotolle. Kehittämisen kohteena oli kiireettömän vastaanoton palveluprosessi. Projektin tavoitteena oli mahdollistaa asiakasosallisuus hukkatekijöiden tunnistamisessa ja yksikön kehittämistoiminnassa kehittämistä ohjaavan ajattelumallin mukaisesti.
Projekti toteutettiin kolmivaiheisena kehittämistutkimuksena. Kehitettävän mittarin sisällön perustaksi toteutettiin asiakasnäkökulmaa korostava systemoitu kirjallisuuskatsaus (N=481) terveydenhuollon hukkatekijöistä. Lopullinen aineisto (n=25) analysoitiin teorialähtöisesti. Kirjallisuuskatsauksen tulokset sovellettiin kehitettävään palveluprosessiin ja tämän sovelluksen perusteella laadittiin asiakaskäyttöön soveltuva mittari hukkatekijöiden tunnistamiseen. Kehitetyn mittarin toimivuutta varmistettiin asiantuntija-arvioon (n=6) perustuvan luotettavuusindeksin, testaamisen (n=19) sekä testivastaajien haastattelun avulla (n=8).
Kehittämisprojektissa tuotettiin helppo- ja nopeakäyttöinen mittari terveydenhuollon hukkatekijöiden määrän ja muodon tunnistamiseen. Asiantuntija-arvioon sekä haastatteluihin perustuen mittarin sisällön laatimisessa onnistuttiin hyvin. Mittari soveltuu prosessin toistuvaan arviointiin. Se on muokattavissa tarpeita vastaavaksi sekä muunneltavissa prosessin eri vaiheisiin. In an effort to respond to the resource and cost pressures arising from a transformation of the population structure, the role of clients in developing health care activities should be expanded. Lean development has been employed in health care to seek solutions to the challenges related to costs and resources. However, shortcomings have also been detected in developing activities in accordance with this ideology in client involvement and the approaches enabling client inclusion.
The purpose of this development project was to develop an indicator suitable for use by clients in identifying waste factors at the appointments by physicians and nurses in the Uusikaupunki social and health centre. The target of development was the service process for non-urgent appointments. The aim of the project was to enable client inclusion in identifying waste factors and development activities at the health care unit in line with the ideology guiding the development.
The project was implemented as a three-stage development study. To serve as the basis for the developed indicator, a systematic literature review (N=481) was carried out on the waste factors in health care with an emphasis on the client perspective. The final data (n=25) were analysed with a theory-guided approach. The results of the literature review were applied to the service process under development, and this application was used as the basis for preparing an indicator for identifying waste factors suitable for use by clients. The functionality of the developed indicator was ensured with a reliability index based on specialist assessment (n=6), testing (n=19), and interviews of test respondents (n=8).
The development project produced an indicator for easy and quick identification of the number and forms of waste factors in health care. Based on the specialist assessment and interviews, the creation of content for the indicator had been successful. The indicator is suitable for the recurrent assessment of the process. It can be modified to correspond to needs and adapted to the various phases of the process.
